不同角膜直径对Pentacam及Corvis ST生物力学结果检测的影响OACSTPCD
The influence of different corneal diameters on Pentacam systemcombined with Corvis ST
目的 探讨不同的角膜直径(WTW)对Pentacam角膜地形图中的Belin/Ambrósio增强扩张显示(BAD)系统以及Corvis角膜生物力学结果的影响.方法 回顾性病例研究.收集2021年1~6月于我院屈光手术中心进行屈光手术术前检查的近视患者132例(132只眼),其中男性72例(72只眼),女性60例(60只眼);年龄(25.41±6.12)岁,并且所有患者均排除了圆锥角膜的可能性.上述患者按照其角膜直径大小进行分组,其中A组角膜直径≤11.5 mm,共66只眼;B组角膜直径≥11.6 mm,共66只眼.所有患者均采用Pentacam角膜地形图检测角膜最薄点厚度(CCT)以及BAD角膜扩张分析功能检测角膜前表面高度偏差值(Df)、后表面高度偏差值(Db)、平均厚度进展偏差值(Dp)、最薄点厚度偏差值(Dt)以及Ambrósio厚度最大变化率偏差值(Da)和总偏差值(Do),Corvis ST角膜生物力学形变幅度比值(DA Ratio)、最薄点厚度/厚度变化率(ARTH)、硬度参数(SPA1)、Corvis生物力学指数(CBI),断层地形图联合生物力学指数(TBI)等参数.术前2个组的角膜地形图以及生物力学指标组间比较均使用单因素方差分析以及独立样本t检验.结果 A、B两组Pentacam系统相关指标结果分别为CCT:533.38±31.16、552.66±31.55;Df:1.02±0.99、0.92±1.46;Db:1.74±1.97、0.61±1.15;Dp:1.67±0.75、1.27±1.34;Dt:-0.37±0.86、0.18±0.89;Da:0.63±0.62、0.78±0.76;Do:1.70±0.51、1.32±0.85.A、B两组CorvisST系统关指标结果分别为DA Ratio:4.14±0.40、4.14±0.46;ARTH:437.04±76.60、470.46±103.36;SPA1:99.80±17.27、96.31±17.36;CBI:0.36±0.18、0.30±0.21;TBI:0.42±0.21、0.44±0.28.经过独立样本t检验得出,两组间的Dt、Db、Dp、CCT、Do等形态学参数差异均有统计学意义(均P<0.05),此外生物力学参数中ARTH值在两组间也具有统计学意义(P<0.05).结论 角膜直径大小对BAD系统以及Corvis ST系统的分析结果有影响.
Objective To explore the effect of differentcorneal diameter on Belin/Ambrósio enhanced ectasia dis-play(BAD)and Corvis ST.Methods Retrospective case study.This study recruited 132 eyes(72 males and 60 fe-males),who had completed preoperative examination before corneal refractive surgery at our center from January to June 2021.Age was(25.41±6.12)years old and No patients had keratoconus.All the patients recruited were divided into two groups according to the corneal diameter measured by the Pentacam system(WTW).Group A included 66 eye swith corneal diameter≤11.5 mm;group B also included 66 eyes whose corneal diameter is≥11.6 mm.Central corneal thickness(CCT),the deviation of front elevation difference map(Df),deviation of back elevation difference map(Db),deviation of average pachymetric progression index(Dp),deviation of minimum thickness(Dt),deviation of Ambrósio′s relational thickness maximum(Da)and overall deviation value(Do)of BAD parameters were measured by Pentacam.The Corvis ST parameters included Integr.Radius,corneal thickness thinnest/pachymetric progression(ARTh),the ratio of deformation amplitude(DA ratio),resultant pressure divided by deflection amplitude at the first applanation(SPA1),the Corvis Bio-mechanical Index(CBI)and tomographic and biomechanical index(TBI).The preoperative BAD parameters and Corvis ST parameters between these two groups were analyzed with independent sample T test.Results In groupsA and B,CCT was 533.38±31.16 and 552.66±31.55;Df was 1.02±0.99 and 0.92±1.46;Db was 1.74±1.97 and 0.61±1.15;Dp was 1.67±0.75 and 1.27±1.34;Dt was-0.37±0.86 and 0.18±0.89;Da was 0.63±0.62 and 0.78±0.76;Do was 1.70±0.51 and 1.32±0.85.DA Ratio was 4.14±0.40 and 4.14±0.46;ARTH was 437.04±76.60 and 470.46±103.36;SPA1 was 99.80±17.27 and 96.31±17.36;CBI was 0.36±0.18 and 0.30±0.21;TBI was 0.42±0.21 and 0.44±0.28,respectively.The BAD param-eters of Dt,Db,Dp,CCT and Do were all significantly different between these two groups(all P<0.05).The Corvis param-eters of ARTH were also significantly different between these two groups(P<0.05).Conclusions Different corneal diam-eter had a crucial influence on BAD and Corvis ST.
