基于健康行为促进模型胃肠肿瘤合并肌少症病人围术期运动方案的构建与实施
Construction and implementation of perioperative exercise program for gastrointestinal cancer patients with sarcopenia based on health behavior promotion model

Abstract
Objective:To develop an exercise training program for perioperative management of middle-aged and elderly patients with sarcopenia associated with gastrointestinal tumors and to explore its implementation effects.Methods:Based on the Health Action Process Approach(HAPA)theoretical framework,an exercise intervention program was developed for middle-aged and elderly patients undergoing perioperative management for gastrointestinal tumors.A total of 104 patients with sarcopenia associated with gastrointestinal tumors from the general surgery department of a tertiary hospital in Tangshan were randomly divided into a control group and an intervention group,with 52 patients in each.The control group received routine perioperative exercise guidance,while the intervention group received additional exercise management based on the HAPA model.Assessments of sarcopenia status,postoperative recovery progress,and quality of life were conducted before the intervention,at 4 weeks,and at 12 weeks after the intervention for both groups.Results:A total of 52 patients in the control group and 50 patients in the intervention group completed the study.The intervention group showed improvements in grip strength,first bowel movement time,drainage tube removal,postoperative hospital stay,exercise self-efficacy,and quality of life compared with the control group(P<0.05).Conclusion:The exercise program based on the HAPA model can enhance muscle strength and physical function,delay the progression of sarcopenia,promote postoperative recovery,and improve the quality of life of middle-aged and elderly patients undergoing perioperative management for gastrointestinal tumors.关键词
