Abstract
Objective To analyze the clinical manifestations of infectious mononucleosis(IM)in different heat duration of children with Epstein-Barr virus(EBV)infection. Methods A retrospective analysis was conducted on 90 children with IM caused by EBV accompanied by fever admitted to Xuzhou Maternity and Child Health Care Hospital from August 2021 to August 2024.According to different heat duration, the children were divided into heat duration>7 days group(22 cases)and heat duration≤7 days group(68 cases). The clinical manifestations of the two groups at admission were analyzed, the laboratory indexes of the two groups were compared, and logistic regression model was used to analyze the influencing factors of prolonged heat duration in children. Results Compared with heat duration≤7 days group, the heat peak, the incidence of hepatomegas, the proportion of monocytes, the proportion of suppressor T cell(Ts),the levels of alanine aminotransferase, lactate dehydrogenase and immunoglobulin G in heat duration>7 days group were higher(P<0.05),but the ratio of helper T cell(Th)/Ts and the proportion of Th were lower(P<0.05).Multivariate logistic regression analysis showed that liver enlargement and Th/Ts were risk factors for prolonged heat course in children with IM(P<0.05).Conclusion The clinical manifestations of IM children caused by EBV infection is different and the heat duration is also different.Children with longer heat duration have higher heat peak, more prone to hepatomegaly, and more prone to damage of immune system function.Hepatomegaly and low Th/Ts are risk factors for prolonged heat duration in children with IM caused by EBV.关键词
