血清PCT、CRP与HMGB1对重症肺炎合并呼吸衰竭患者的预后预测价值OA
Prognostic Value of Serum PCT,CRP and HMGB1 in Patients with Severe Pneumonia Complicated with Respiratory Failure
目的:探讨血清降钙素原(PCT)、C反应蛋白(CRP)与高迁移率族蛋白B1(HMGB1)对重症肺炎合并呼吸衰竭患者的预后预测价值,方法:选取2019年1月—2023年12月厦门医学院附属第二医院收治的116例重症肺炎合并呼吸衰竭患者,均行血清PCT、CRP、HMGB1水平检测,根据入院后28 d预后情况分为预后良好组(n=80)和预后不良组(n=36).分析影响患者预后的相关因素以及血清PCT、CRP、HMGB1的预测价值.结果:预后不良组氧合指数低于预后良好组,急性生理和慢性健康状况(APACHE Ⅱ)评分以及血清PCT、CRP与HMGB1水平高于预后良好组,差异有统计学意义(P<0.05).多因素logistic回归分析显示,APACHE Ⅱ评分(OR=3.789)以及血清PCT(OR=5.259)、CRP(OR=5.936)、HMGB1(OR=5.181)是影响重症肺炎合并呼吸衰竭患者预后的危险因素.ROC曲线分析显示,血清PCT、CRP、HMGB1联合检测对重症肺炎合并呼吸衰竭患者预后的预测AUC为0.970,敏感度为91.7%,特异度为93.7%,均较各单项指标检测更高.结论:血清PCT、CRP与HMGB1对重症肺炎合并呼吸衰竭患者的预后均具有一定的预测价值,且联合应用的预测效能更高.
Objective:To investigate the prognosticative value of serum procalcitonin(PCT),C-reactive protein(CRP)and high mobility group protein B1(HMGB1)in patients with severe pneumonia complicated with respiratory failure.Method:116 patients with severe pneumonia complicated with respiratory failure admitted to Second Affiliated Hospital of Xiamen Medical College from January 2019 to December 2023 were selected as the study objects.Serum PCT,CRP and HMGB1 levels were detected in all patients,and the patients were divided into good prognosis group(n=80)and poor prognosis group(n=36)according to the prognosis 28 days after admission.To analyse the relevant factors affecting patients'prognosis and the predictive value of serum PCT,CRP and HMGB1.Result:The oxygenation index of the poor prognosis group was lower than that of the good prognosis group,and the APACHE Ⅱ score,serum PCT,CRP and HMGB1 levels were higher than those of the good prognosis group,and the difference is statistically significant(P<0.05).Analysis showed that APACHE Ⅱ score(OR=3.789),serum PCT(OR=5.259),CRP(OR=5.936)and HMGB1(OR=5.181)were risk factors affecting the prognosis of patients with severe pneumonia complicated with respiratory failure.ROC curve analysis showed that the combined detection of serum PCT,CRP and HMGB1 predicted the prognosis of patients with severe pneumonia complicated with respiratory failure with an AUC of 0.970,a sensitivity of 91.7%and a specificity of 93.7%,which were higher than the detection of each single index.Conclusion:Serum PCT,CRP and HMGB1 can predict the prognosis of patients with severe pneumonia complicated with respiratory failure,and the combined application is more effective.
