Abstract
Objective:To investigate the changes in blood potassium,matrix metalloproteinase-9(MMP-9),and free triiodothyronine/free thyroxine(FT3/FT4)ratio in patients with acute heart failure(AHF)and their relationship with prognosis.Method:118 AHF patients admitted to Yidu People's Hospital from August 2021 to August 2023 were selected as the AHF group,and 56 healthy adults who underwent physical examinations during the same period were used as controls.The blood potassium,MMP-9 and FT3/FT4 ratio were compared,and the correlation between each indicator and the severity of AHF disease were analyzed.All AHF patients were followed up for 1 year to analyze the predictive value of blood potassium,MMP-9,and FT3/FT4 ratio on patient prognosis.Result:The serum potassium,FT3,and FT4 levels in the AHF group were lower than those in the control group,the FT3/FT4 ratio in the AHF group was lower than that in the control group,and the MMP-9 level in the AHF group was higher than that in the control group(P<0.05).The serum potassium and FT3 levels of New York Heart Association(NYHA)grade Ⅳ patients were lower than those of grade Ⅰ-Ⅱ and Ⅲ patients,and grade Ⅲ patients were lower than those of grade Ⅰ-Ⅱ patients(P<0.05).The FT3/FT4 ratio of NYHA grade Ⅳ patients was lower than that of grade Ⅰ-Ⅱ and Ⅲ patients,and grade Ⅲ patients were lower than those of grade Ⅰ-Ⅱ patients(P<0.05).The MMP-9 levels in NYHA grade Ⅳ patients were higher than those in grade Ⅰ-Ⅱ and Ⅲ patients,and grade Ⅲ patients were higher than those in grade Ⅰ-Ⅱ patients(P<0.05).The blood potassium level and FT3/FT4 ratio of AHF patients are negatively correlated with NYHA cardiac function grading,while MMP-9 level is positively correlated with NYHA cardiac function grading(r=-0.649,-0.525,0.673,P<0.05).The serum potassium,FT3,and FT4 levels in the poor prognosis group were lower than those in the good prognosis group.The FT3/FT4 ratio was lower than that in the good prognosis group,and the MMP-9 levels were higher than those in the good prognosis group(P<0.05).The combined detection of various indicators predicted the AUC value of AHF patients,which was higher than MMP-9 and the FT3/FT4 ratio detected separately(P<0.05).Conclusion:The changes in blood potassium,MMP-9,and FT3/FT4 ratio in AHF patients are correlated with the severity of the disease,and the combined detection of various indicators has predictive value for patient prognosis.关键词
