目的:分析富血小板血浆联合负压封闭引流对深Ⅱ度烧伤患者肉芽组织相关因子、炎症反应的影响.方法:选取 2019 年 8 月—2023 年 9 月宁夏医科大学总医院收治的 61 例深Ⅱ度烧伤患者作为研究对象,采用随机数表法分为对照组 30 例,观察组 31 例.对照组接受负压封闭引流治疗,观察组在此基础上接受富血小板血浆治疗,均治疗至创面愈合.比较两组各项指标.结果:观察组治疗 14 d的临床总有效率高于对照组,差异有统计学意义(P<0.05);两组治疗 14 d后的表皮生长因子(EGF)、碱性成纤维细胞生长因子(bFGF)、胰岛素样生长因子(IGF)、血小板衍生生长因子(PDGF)水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);两组治疗 1 个月后的C反应蛋白(CRP)、二胺氧化酶(DAO)、白细胞(WBC)水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05).结论:富血小板血浆联合负压封闭引流可上调深Ⅱ度烧伤患者肉芽组织相关因子的表达,促进创面愈合,减轻其炎症反应,疗效确切.

Objective:To analyze the effect of platelet-rich plasma combined with negative pressure sealing drainage on granulation tissue related factors and inflammatory reaction in patients with deep second-degree burn.Method:61 patients with deep second-degree burn admitted to the General Hospital of Ningxia Medical University from August 2019 to September 2023 were selected as the study objects and divided into the control group(30 cases)and the observation group(31 cases)by random number table method.The control group received negative pressure closed drainage treatment,while the observation group received platelet-rich plasma treatment on the basis of this,and both groups were treated until wound healing.The indexes of the two groups were compared.Result:The clinical total effective rate of the observation group was higher than that of the control group after 14 days of treatment(P<0.05).The levels of epidermal growth factor(EGF),basic fibroblast growth factor(bFGF),insulin-like growth factor(IGF)and platelet derived growth factor(PDGF)after 14 days of treatment were higher than those before treatment,and the observation group was higher than control group,and the difference was statistically significant(P<0.05).The levels of C-reaction protein(CRP),diamine oxidase(DAO)and white blood cell(WBC)in serum after 1 month of treatment were lower than those before treatment,and the the observation group was higher than control group(P<0.05).Conclusion:Platelet-rich plasma combined with negative pressure sealing drainage can up-regulate the expression of granulation tissue-related factors in patients with deep second-degree burn,promote wound healing and reduce its inflammatory reaction,with definite curative effect.
