生酮饮食对脑瘫共患癫痫患儿肠道菌群、粗大运动功能和癫痫发作的影响
Effect of ketogenic diet on gut microbiota,gross motor function and epileptic seizures in children with cerebral palsy and epilepsy

Abstract
Objective To observe the effect of ketogenic diet(KD)on gut microbiota,gross motor function and epileptic sei-zures in children with cerebral palsy and epilepsy.Methods Forty-two children with cerebral palsy and epilepsy admitted to the hospital from January 2022 to January 2024 were selected.Using the random number table method,the subjects were divided into a control group and an observation group,with 21 cases in each group.The control group was given convention-al drug treatment and rehabilitation treatment.On this basis,the observation group adopted KD.Both groups were treated for 24 weeks.The changes in gut microbiota,Gross Motor Function Measure(GMFM-88)scores,Gesell Developmental Scale scores,physical development and amount of antiepileptic drugs(AEDs)before and after treatment were compared be-tween the two groups.Epileptic seizure control,electroencephalogram(EEG)outcomes and the incidence of adverse reac-tions during treatment were statistically analyzed.Results After 24 weeks of treatment,the abundance of Bacteroidetes,Actinobacteria,Verrucomicrobia,Clostridia,Erysipelothrix rhusiopathiae and Coriobacteriia in the observation group was higher than that in the control group(P<0.05).The abundance of Fusobacteria and beta-Proteobacteria was lower than that in the control group(P<0.05).The GMFM-88 scores,Gesell scores in different dimensions,total effective rates of epileptic seizure control and EEG epileptic wave control in the observation group were higher than those in the control group after 24 weeks of treatment(P<0.05).There was no significant difference in growth level,fitness and malnutrition grades of the two groups before treatment and after 12 and 24 weeks of treatment(P>0.05).The amount of AEDs in the two groups was comparable at the same time point(P>0.05).The incidence rates of adverse reactions in the two groups were close(P>0.05).Conclusion KD can effectively improve gut microbiota,gross motor function and development quotient of children with cerebral palsy and epilepsy.It is also helpful for epileptic seizure,with high safety.关键词
