纤维支气管镜吸痰联合肺泡灌洗治疗重症肺炎的疗效及对近期预后的影响
Effect of sputum suction and alveolar lavage by fiberoptic bronchoscope in the treatment of severe pneumonia and the impact on short-term prognosis

Abstract
Objective To investigate the effect of sputum suction and alveolar lavage by fiberoptic bronchoscope in the treat-ment of severe pneumonia,and analyze the impact on short-term prognosis.Methods A total of 98 patients with severe pneumonia were randomly divided into two groups.All patients were given conventional symptomatic treatment.Patients in the control group(n=49)were treated with sputum suction by closed sterile suction tube,while patients in the observation group(n=49)were treated with sputum suction and alveolar lavage by fiberoptic bronchoscope.Clinical efficacy,arterial blood gas indexes,inflammatory factors,short-term prognosis and the incidence of adverse reactions were compared be-tween the two groups.Results The overall clinical efficacy in the observation group was better than that in the control group(P<0.05).After 7 days of treatment,there were statistically significant differences in partial pressure of blood oxy-gen(PaO2),blood oxygen saturation(SaO2),oxygenation index(PaO2/FiO2),white blood cell count(WBC),interleukin-6(IL-6),interleukin-8(IL-8),interferon-γ(IFN-γ)and APACHE II scores between the two groups(P<0.05).The du-ration of mechanical ventilation and length of ICU stay in the observation group were shorter than those in the control group(P<0.05).The incidence rates of adverse reactions in the two groups were close(P>0.05).Conclusion Sputum suction and alveolar lavage by fiberoptic bronchoscope can improve the clinical efficacy in patients with severe pneumonia,improve arterial blood gas indexes,alleviate the damage of lung tissue due to the inflammatory factors and accelerate the recovery process,thus improving short-term prognosis.Besides,it is safe.关键词
