Abstract

Objective To investigate the relationship between serum homocysteine(Hcy),C-reactive protein/albumin ratio(CAR)and the disease and outcome of bronchophenitis in children.Methods 102 children with bronchopneumonia admitted to our hospital from May 2022 to May 2024 were selected as the subjects of this study,and were divided into mild group(73 cases)and severe group(29 cases)according to their disease conditions.After 1 course of treatment,the children with bronchopneumonia were divided into a good outcome group(85 cases)and a bad outcome group(17 cases).Serum levels of Hcy,C-reactive protein(CRP)and albumin(ALB)were detected,and CAR was calculated.The predictive value of serum Hcy and CAR in the prognosis of children with bronchial pneumonia was evaluated by ROC.The factors affecting the out-come of children with bronchopneumonia were investigated by multivariate Logistic stepwise regression analysis;Using ROC curve to evaluate the predictive value of regression model for the outcome of children with bronchopneumonia.Results The serum Hcy[(21.05±3.56)μ mol/L vs(14.72±2.31)μ mol/L]and CAR[(1.45±0.11)vs(0.75±0.04)]levels in the severe group were higher than those in the mild group(P<0.05).Serum Hcy and CAR levels in the poor outcome group were higher than those in the good outcome group(P<0.05).ROC curve analysis showed that the area under the curve(AUC)(95%CI)of serum Hcy and CAR to predict the outcome of children with bronchial pneumonia were 0.757(0.704-0.810)and 0.869(0.816-0.922),respectively,and the cutoff values were 19.24μmol/L and 1.17,respectively.The specificity and sensitivity were 55.93%and 67.64%,respectively,and 93.05%and 93.05%.The combined prediction AUC(95%CI)was 0.902(0.849-0.955),the specificity was 87.12%,and the sensitivity was 85.25%.Multivariate a-nalysis showed that young age(OR=0.457,95%CI:0.318-0.657),long course of disease(OR=2.282,95%CI:1.560-3.338)and severe disease(OR=2.570,95%CI:1.710~3.864),elevated Hcy level(OR=2.465,95%CI:1.649-3.683)and elevated CAR level(OR=3.047,95%CI:1.949-4.763)were independent risk factors for adverse out-comes in children with bronchopneumonia(P<0.05).The AUC(95%CI)of the regression model for predicting the out-come of children with bronchopneumonia was 0.881(0.829-0.933),with a specificity of 67.45%and a sensitivity of 91.69%.Conclusion Elevated serum Hcy and CAR levels are closely related to the severity and outcome of bronchopneumonia in children,and they can be used as biomarkers to predict the outcome of bronchopneumonia in children.
