支气管哮喘患者血清sCD74、LBP水平与急性发作期的关系
The relationship between serum sCD74,LBP and acute attack in patients with bronchial asthma

Abstract
Objective To investigate the relationship between serum soluble CD74(sCD74)and lipopolysaccharide binding protein(LBP)levels and the occurrence of acute attacks in patients with bronchial asthma.Methods From January 2021 to December 2023,74 patients with acute attack of bronchial asthma and 46 patients with remission of bronchial asthma admit-ted to our hospital were included in the acute group and the delayed group,respectively,and 60 healthy subjects were select-ed to set up a control group.Serum sCD74 and LBP levels of all subjects were detected at admission,and the correlation be-tween serum sCD74 and LBP and pulmonary function indexes was analyzed.The influencing factors of the occurrence of a-cute attacks in patients with bronchial asthma were analyzed by multivariate Logistic regression.ROC was drawn to evaluate the value of serum sCD74 and LBP in the occurrence of acute attacks in patients with bronchial asthma.Results Serum levels of sCD74 and LBP in acute group were higher compared with remission group and control group,and those in remission group were higher than those in control group(sCD74:72.58±7.14 vs 85.01±8.67 vs 105.33±13.58)(LBP:15.32±1.86 vs 18.24±2.91 vs 24.37±4.18)(F=9.577、14.573,P<0.05).FEV1/FVC and PEF%pred in control group were higher compared with remission group and acute group,and those in remission group were higher compared with acute group(FEV1/FVC:65.76±6.86 vs 73.52±8.13 vs 88.74±10.02)(PEF%pred:70.08±8.27 vs 80.46±13.22 vs 5.13±9.13)(F=9.404、13.991,P<0.05).The serum levels of sCD74 and LBP in severe group were higher compared with mild and moderate groups,and the moderate group was higher compared with mild group(sCD74:72.58±7.14 vs 85.01±8.67 vs 105.33±13.58)(LBP:15.32±1.86 vs 18.24±2.91 vs 24.37±4.18)(F=9.577、14.573,P<0.05).Serum sCD74(r=-0.583,-0.529,P<0.05)and LBP(r=-0.489,-0.602,P<0.05)were negatively correlated with FEV1/FVC and PEF%pred in patients with acute bronchial asthma.Multivariate analysis showed that the lower the FEV1/FVC ratio(OR=0.631,95%CI:0.428-0.878),the lower the PEF%pred(OR=0.597,95%CI:0.402-0.887),the higher the serum sCD74 level(OR=2.787,95%CI:1.591-4.882),and the higher the serum LBP level(OR=3.846,95%CI:1.933-7.652),the greater the risk of acute attacks in patients with bronchial asthma.The AUC(95%CI)of serum sCD74,LBP and their combination during acute attack were 0.763(0.718-0.813),0.821(0.771-0.866)and 0.904(0.859-0.954),respectively.Conclusion Serum sCD74 and LBP levels are highly expressed in patients with bronchial asthma,and there is a significant correlation with the occurrence and severity of acute attacks,which may be an effective in-dicator to predict the acute attacks of asthma.关键词
