基于相火理论分期论治糖尿病肾病
Treatment of diabetic nephropathy from perspective of ministerial fire
摘要
Abstract
Diabetic nephropathy(DN)refers to chronic kidney disease caused by diabetes,with a complex pathogenesis and challenging clinical treatment.Hyperactivity of ministerial fire is the core mechanism of DN.Based on the theory of ministerial fire,a staged approach can be adopted for the treatment of DN.In the early stage,the pathogenesis involves spleen yang deficiency,phlegm-dampness obstruction,gallbladder fire rebelling upwards,and depletion of body fluids.The treatment should focus on clearing and descending ministerial fire,boosting qi,and nourishing fluids.In the middle stage,the pathogenesis involves spleen and kidney yang deficiency,cold-dampness descending,ministerial fire stabilizing,and a combination of blood stasis and water retention.The treatment should focus on warming and descending ministerial fire,promoting diuresis,and eliminating dampness.In the late stage,the pathogenesis involves a decline in the fire of the gate of vitality,floating ministerial fire,widespread damp-turbidity,and toxic damage to the kidney collaterals.The treatment should focus on warming and tonifying ministerial fire,removing toxins,and dissolving turbidity.Clinical case studies are presented to elaborate on the staged treatment of DN based on the theory of ministerial fire.关键词
