The effects of combined diet-exercise interventions on prediabetes:A systematic review and meta-analysis
目的:通过系统评价饮食运动联合干预对糖尿病前期人群的干预效果,为糖尿病前期人群的疾病预防提供证据.方法:检索中国知网、维普数据库、万方数据库、The Cochrane Library、Pubmed、Web of Science、Embase等数据库,检索时间为从建库到2024年3月选择关于糖尿病前期与饮食运动干预有关的随机对照试验,对纳入的文献进行Meta分析,使用Revman5.4软件进行统计分析.结果:最终纳入24篇文献,Meta分析结果显示:饮食运动联合干预对糖尿病前期人群的糖尿病发病率(RR=0.61,95%CI:0.51,0.72,P<0.001)、空腹血糖(MD=-0.30,95%CI:-0.59,-0.01,P=0.04)、糖化血红蛋白(MD=-0.25,95%CI:-0.40,-0.10,P=0.001)、餐后 2 h血糖(MD=-0.78,95%CI:-1.41,-0.15,P=0.02)、BMI(MD=-0.69,95%CI:-1.16,-0.22,P=0.004)具有改善效果.亚组分析结果表明在年龄>55岁人群中相较于年龄≤55岁人群干预效果更好;但长时间(>12个月)的干预效果有待进一步验证.结论:生活方式干预能够有效降低糖尿病前期人群糖尿病发病率、空腹血糖、糖化血红蛋白、餐后2 h血糖、BMI水平,延缓糖尿病的进展.
Objective:This systematic review aims to evaluate the effects of combined diet-exercise modifications on individu-als with prediabetes,aiming to provide compelling evidence for the prevention of prediabeteic conditions.Method:The search was conducted across multiple databases,including CNKI,VIP Database,Wanfang Database,the Cochrane Library,PubMed,Web of Science,and Embase.The search period encompassed the entire duration of the databases,starting from their inception and ex-tending until March 2024.Randomized controlled trials pertaining to prediabetes and combined diet-exercise were specifically cho-sen for inclusion.The included literature was subjected to a meta-analysis using Revman 5.4 software for statistical analysis.Re-sults:A total of 24 studies were included in the meta-analysis,which revealed that combined diet-exercise interventions significant-ly reduced the incidence of diabetes(RR=0.61,95%CI:0.51,0.72,P<0.001),fasting blood glucose(MD=-0.30,95%CI:-0.59,-0.01,P=0.04),glycated hemoglobin(MD=-0.25,95%CI:-0.40,-0.10,P<0.001),postprandial 2-hour blood glucose(MD=-0.78,95%CI:-1.41,-0.15,P=0.02),and BMI(MD=-0.69,95%CI:-1.16,-0.22,P=0.004),all demonstrating improvements as a result of these interventions.The subgroup analysis findings suggested that the intervention was more effective in individuals aged over 55 compared to those aged 55 or younger.However,further validation is required to con-firm the efficacy of longer-term interventions(>12 months).Conclusion:Combined diet-exercise interventions have been shown to effectively reduce the incidence of diabetes,fasting blood glucose levels,glycated hemoglobin levels,postprandial 2-hour blood glucose levels,and BMI among individuals in the prediabetic stage,thereby delaying the progression to diabetes.
