The Role and Impact of Multidisciplinary Collaboration on the Efficiency of Chronic Disease Management

Abstract
Multidisciplinary collaboration provides comprehensive and personalized treatment solutions for chronic patients through the collaboration of experts from different disciplines.This model not only pays attention to the disease itself,but also the psychological,living habits and social environment of patients,so as to develop comprehensive health management programs.Multidisciplinary collabora-tion is helpful to timely adjust the treatment strategy of patients,improve the treatment effect and the quality of life of patients,and have a positive impact on the efficiency of clinical management of chronic diseases.Hospitals need to provide necessary support for the development of multidisciplinary collaboration,strengthen the research and practice of multidisciplinary collaboration model,and con-stantly improve and optimize the model to better meet the treatment and needs of chronic patients.关键词
