基于多模式影像技术观察针刺对卒中后肢体功能障碍的影响OA
Effect of acupuncture on post-stroke limb dysfunction based on multimodal imaging technique
目的 通过多模式影像技术观察针刺对卒中后肢体功能障碍的影响.方法 选取经头颅MRI证实为缺血性卒中患者 116 例,随机分为针刺组(60 例)和康复组(56 例).康复组予脑血管疾病二级预防药物和神经内科常规治疗,并配合肢体功能康复训练;针刺组在康复组治疗基础上另予针刺治疗.比较两组治疗前后Brunnstrom 分期变化、头颅弥散张量成像技术(diffusion tensor image,DTI)中的表观弥散系数(apparent diffusion coefficient,ADC)值和各向异性值(fractional anisotropy,FA)以及脑灌注成像检测梗死面积和侧支循环情况.结果 治疗后,针刺组Brunnstrom分期优于康复组(P<0.05).两组治疗后ADC值均低于治疗前(P<0.05),FA 值均高于治疗前(P<0.05).两组治疗后梗死面积较治疗前没有明显进展(P>0.05),略微减小但不明显.两组治疗后侧支循环明显改善(P<0.05),针刺组治疗后侧支循环优于康复组(P<0.05).结论 在常规治疗与康复训练基础上,针刺可能通过刺激大脑初级皮层活跃,促使大脑血流增加,尤其增加梗死周围侧支循环血流,加强梗死区域运动皮层与相关脑区的连通性,进一步激活皮质纤维束功能,从而改善卒中后肢体功能.
Objective To observe the effect of acupuncture on post-stroke limb dysfunction by multimodal imaging technique.Method A total of 116 patients with ischemic stroke confirmed by cranial MRI were selected and randomly divided into an acupuncture group(60 cases)and a rehabilitation group(56 cases).The rehabilitation group was treated with secondary prevention drugs for cerebrovascular diseases and routine neurological treatment,and was accompanied by limb function rehabilitation training;the acupuncture group was treated with acupuncture on the basis of the rehabilitation group.The Brunnstrom stage changes,apparent diffusion coefficient(ADC)and fractional anisotropy(FA)in diffusion tensor image(DTI)and cerebral perfusion imaging to detect infarct area and collateral circulation were compared between the two groups before and after treatment.Result After treatment,Brunnstrom staging was better in the acupuncture group than in the rehabilitation group(P<0.05).The ADC value of both groups was lower than that before treatment(P<0.05),and the FA value of both groups was higher than that before treatment(P<0.05).The infarct area of both groups did not progress significantly compared with that before treatment(P>0.05),decreased slightly but not significantly.The collateral circulation of both groups improved significantly after treatment(P<0.05),and the collateral circulation of the acupuncture group was better than that of the rehabilitation group after treatment(P<0.05).Conclusion On the basis of conventional treatment and rehabilitation training,acupuncture may improve limb function after stroke by stimulating the primary cortex to be active,increasing cerebral blood flow,especially increasing peri-infarct collateral circulation,strengthening the connectivity between the motor cortex in the infarcted area and the related brain areas,and further activating the function of the cortical fiber tracts.
