目的 基于基因表达综合(GEO)数据库数据,探讨微小RNA-20a-5p(miR-20a-5p)和E2F转录因子 3(E2F3)在心力衰竭(HF)患者中的表达情况及临床意义.方法 在GSE136547 和GSE104150 数据集中筛选差异表达微小RNA(miRNA)及其靶基因信使RNA,利用Cytoscape分析差异表达的关键miRNA.选取 2020 年 1 月至 2020 年 12 月在南通大学附属常熟医院就诊的HF患者80例为HF组,同期左心室收缩功能正常的80例体检者为对照组.收集并比较受试者临床资料.采用qPCR法验证差异表达的关键miRNA及其靶基因在HF组及对照组的表达情况.ROC曲线分析差异表达的关键miRNA及其靶基因对HF诊断价值;Pearson相关分析差异表达的关键miRNA及其靶基因与左心室射血分数(LVEF)、B型利钠肽(BNP)的相关性.结果 GSE136547数据集中发现11个差异表达miRNA,GSE104150中发现170个差异表达miRNA,交集为 6 个.经Cytoscape分析差异表达的关键miRNA为miR-20a-5p、miR-15a-5p,两者靶基因交集为E2F3、ABL原癌基因 2(ABL2)、Kelch样家族成员 15(KLHL15).两组间miR-20a-5p、E2F3 差异有统计学意义(t=4.289、5.854,P<0.05),不同基础疾病HF患者miR-20a-5p、E2F3表达水平差异无统计学意义(P>0.05).ROC曲线分析显示miR-20a-5p和E2F3均对HF有诊断价值,截断值分别为3.906、4.043.miR-20a-5p与BNP呈负相关,与LVEF呈正相关(r=-0.229、0.337,P<0.05);E2F3 与BNP呈正相关,与LVEF呈负相关(r=0.280、-0.277,P<0.05).结论 miR-20a-5p和E2F3 在HF患者中表达异常,两者对HF均有诊断价值.

Objective To investigate the expression and clinical significance of microRNA-20a-5p(miR-20a-5p)and E2F transcription factor 3(E2F3)in heart failure(HF)patients based on data from the Gene Expression Omnibus(GEO)database.Methods Differentially expressed microRNAs(miRNAs)and their target messenger RNAs were screened in the GSE136547 and GSE104150 datasets.Cytoscape was used to analyze key differentially expressed miRNAs.Eighty HF patients treated at the Affiliated Changshu Hospital of Nantong University from January to December 2020 were selected as the HF group,while 80 individuals with normal left ventricular systolic function who underwent routine health examinations during the same period were selected as the control group.Clinical data were collected and compared between the two groups.Key differentially expressed miRNAs and their target genes were validated using qPCR in both groups.Receiver operating characristic(ROC)curve analysis was conducted to assess the diagnostic value of the key differentially expressed miRNAs and their target genes for HF.Additionally,Pearson correlation analysis was performed to evaluate the correlations between the key differentially expressed miRNAs and their target genes and left ventricular ejection fraction(LVEF)and B-type natriuretic peptide(BNP).Results There were 11 differentially expressed miRNAs in the GSE136547 dataset and 170 in the GSE104150 dataset,with 6 overlapping.Two key differentially expressed miRNAs were identified by Cytoscape analysis:miR-20a-5p and miR-15a-5p,with E2F3,ABL proto-oncogene 2(ABL2),and Kelch-like family member 15(KLHL15)as their intersecting target genes.The expressions of miR-20a-5p and E2F3 in HF and control groups were significantly different(t=4.289,5.854;P<0.05),while no significant differences were observed in HF patients with various underlying diseases(P>0.05).The ROC curve analysis showed that both miR-20a-5p and E2F3 had diagnostic value for HF,with cut-offvalues of 3.906 and 4.043,respectively.MiR-20a-5p was negatively correlated with BNP and positively correlated with LVEF(r=-0.229,0.337;P<0.05),whereas E2F3 was positively correlated with BNP and negatively correlated with LVEF(r=0.280,-0.277;P<0.05).Conclusion MiR-20a-5p and E2F3 exhibit abnormal expression in HF patients,and both show diagnostic value for HF.
