心周脂肪组织体积与冠心病患者动脉僵硬度及血清炎症因子的相关性OACSTPCD
Correlation of pericardial adipose tissue volume with arterial stiffness and serum inflammatory factors in patients with coronary heart disease
目的 探讨心周脂肪组织体积(PATV)与冠心病(CHD)患者动脉僵硬度及血清炎症因子的相关性.方法 选取2021年5月至2023年5月期间开滦总医院、唐山市妇幼保健院及乐亭县医院收治的 119 例CHD患者为CHD组,同期选取 60 例健康体检者为对照组,根据CHD组患者肱踝脉搏波传导速度(baPWV)分为轻度僵硬组 68 例和重度僵硬组 51 例.受试者均进行多层螺旋CT扫描,测量PATV.采用动脉硬化检测仪测量baPWV;采用酶联免疫法检测血清炎症因子C反应蛋白(CRP)、基质金属蛋白 9(MMP-9)及脂蛋白相关磷脂酶A2(LpPLA2).比较CHD组与对照组、轻度僵硬组与重度僵硬组间的PATV、baPWV、CRP、MMP-9、LpPLA2 水平,比较轻度僵硬组与重度僵硬组间临床资料.采用Pearson分析法分析CHD患者PATV与baPWV及炎症因子的相关性.采用Logistic回归法分析CHD患者动脉重度僵硬的影响因素.结果 与对照组比较,CHD组PATV、baPWV、CRP、MMP-9 及LpPLA2 水平均较高(t=19.728、23.340、24.716、6.684、9.670,P<0.05).119 例CHD患者中动脉重度僵硬率为42.86%;与轻度僵硬组比较,重度僵硬组PATV、baPWV、CRP、MMP-9及LpPLA2 水平均较高(t=6.141、25.339、8.715、6.176、5.886,P<0.05).相关性分析结果显示,CHD患者PATV与baPWV、CRP、MMP-9 及LpPLA2均呈正相关(r=0.600、0.469、0.504、0.537,P<0.05).重度僵硬组与轻度僵硬组年龄、高血压、糖尿病、高脂血症差异有统计学意义(t/χ2=3.363、5.287、4.615、4.236,P<0.05);Logistic回归分析显示,年龄、高血压、PATV、CRP、MMP-9、LpPLA2 是CHD患者重度僵硬的危险因素(OR=1.815、2.492、2.067、2.261、1.642、1.952,P<0.05).结论 年龄、高血压、PATV、CRP、MMP-9、LpPLA2 是CHD患者动脉重度僵硬的危险因素.
Objective To investigate the correlation of pericardial adipose tissue volume(PATV)with arterial stiffness and serum inflammatory factors in patients with coronary heart disease(CHD).Methods A total of 119 CHD patients admitted to Kailuan General Hospital,Tangshan Maternal and Child Health Hospital,and Leting County Hospital from May 2021 to May 2023 were selected as the CHD group,and 60 healthy individuals undergoing physical examinations during the same period were selected as the control group.Based on brachial-ankle pulse wave velocity(baPWV),the CHD group was further divided into a mild stiffness group(68 cases)and a severe stiffness group(51 cases).All subjects underwent multi-slice spiral CT scanning to measure PATV.Arteriosclerosis detector was used to measure baPWV,and serum inflammatory factors,including C-reactive protein(CRP),matrix metalloproteinase-9(MMP-9),and lipoprotein-associated phospholipase A2(LpPLA2)were detected using enzyme-linked immunosorbent assay(ELISA).The levels of PATV,baPWV,CRP,MMP-9,and LpPLA2 were compared between the CHD group and the control group,as well as between the mild and severe stiffness groups.Clinical data were also compared between the mild group and severe stiffness group.Pearson's correlation analysis was used to examine the relationships between PATV and baPWV as well as inflammatory factors in CHD patients.Logistic regression analysis was applied to identify factors influencing severe arterial stiffness in CHD patients.Results Compared with the control group,the levels of PATV,baPWV,CRP,MMP-9 and LpPLA2 in CHD group were increased(t=19.728,23.340,24.716,6.684,9.670;P<0.05).Among the 119 CHD patients,the rate of severe arterial stiffness was 42.86%.Compared with the mild stiffness group,the levels of PATV,baPWV,CRP,MMP-9 and LpPLA2 were increased in the severe stiffness group(t=6.141,25.339,8.715,6.176,5.886;P<0.05).Pearson's correlation analysis showed that PATV was positively correlated with baPWV,CRP,MMP-9 and LpPLA2 in CHD patients(r=0.600,0.469,0.504,0.537;P<0.05).There were statistically significant differences between the severe and mild stiffness groups in terms of age,hypertension,diabetes,and hyperlipidemia(t/χ2=3.363,5.287,4.615,4.236;P<0.05).Logistic regression analysis showed that age,hypertension,PATV,CRP,MMP-9,and LpPLA2 were risk factors for severe stiffness in CHD patients(OR=1.815,2.492,2.067,2.261,1.642,1.952;P<0.05).Conclusion Age,hypertension,PATV,CRP,MMP-9,and LpPLA2 are risk factors for severe arterial stiffness in CHD patients.
