稳定性心绞痛患者热休克蛋白47、可溶性Fms样酪氨酸激酶1水平与冠状动脉病变程度的相关性OACSTPCD
Correlation of heat shock protein 47 and soluble Fms-like tyrosine kinase 1 levels with the severity of coronary artery disease in patients with stable angina
目的 探讨稳定性心绞痛患者血清中热休克蛋白 47(HSP47)、可溶性Fms样酪氨酸激酶 1(sFlt-1)水平及其与冠状动脉病变程度的相关性.方法 选取青海红十字医院 2021 年5 月至 2023 年 5 月收治的稳定性心绞痛患者 85 例作为稳定性心绞痛组,选取同时期在本院健康体检者 85 例作为对照组.根据冠状动脉狭窄程度将稳定性心绞痛组分为为轻度狭窄组 36 例、中度狭窄组 32 例、重度狭窄组 17 例,根据冠状动脉血管病变支数将稳定性心绞痛组分为三支病变组 28 例、双支病变组 32 例、单支病变组 25 例.使用Gensini评分评估冠状动脉病变程度.采用酶联免疫吸附测定法检测血清中HSP47 和sFlt-1 表达水平并分析.采用Spearman相关分析法分析HSP47 和sFlt-1 表达水平与Gensini评分的相关性;采用多因素Logistic回归分析HSP47和sFlt-1 水平与Gensini评分的关系.结果 与对照组相比,稳定性心绞痛组高血压、高脂血症、总胆固醇、C反应蛋白、甘油三酯、同型半胱氨酸、低密度脂蛋白胆固醇、HSP47、sFlt-1 升高,高密度脂蛋白胆固醇降低(t/χ2=6.024、4.628、2.388、5.380、2.125、5.682、2.169、7.972、9.747、2.460,P<0.05);轻度狭窄组、中度狭窄组、重度狭窄组HSP47 和sFlt-1 水平依次升高(P<0.05),单支病变组、双支病变组、三支病变组HSP47 和sFlt-1 水平依次升高(P<0.05);HSP47、sFlt-1 水平与Gensini评分呈正相关(r=0.538、0.641,P<0.01);多因素Logistic回归分析结果显示,HSP47、sFlt-1是影响Gensini评分的独立影响因素(OR=3.971、3.508,P<0.01).结论 HSP47和sFlt-1在稳定性心绞痛患者血清中升高,均是Gensini评分的独立影响因素,与冠状动脉病变严重程度呈正相关.
Objective To analyze the serum levels of heat shock protein 47(HSP47)and soluble Fms-like tyrosine kinase 1(sFlt-1)in patients with stable angina and their correlation with the severity of coronary artery disease.Methods A total of 85 patients with stable angina treated in Qinghai Red Cross Hospital from May 2021 to May 2023 were selected as the stable angina group,and 85 healthy individuals who underwent physical examinations from the same hospital during the same period were regarded as the control group.Based on the degree of coronary artery stenosis,the stable angina group was divided into a mild stenosis group(36 cases),a moderate stenosis group(32 cases),and a severe stenosis group(17 cases).Additionally,based on the number of diseased coronary vessels,the stable angina group was divided into a three-vessel disease group(28 cases),a two-vessel disease group(32 cases),and a single-vessel disease group(25 cases).Serum expression levels of HSP47 and sFlt-1 were detected by enzyme-linked immunosorbent assay(ELISA),and compared among groups.The severity of coronary artery disease was assessed using the Gensini score,and the correlations between HSP47 and sFlt-1 levels and the Gensini score were analyzed using Spearman's correlation.Multiple logistic regression analysis was used to analyze the relationships between serum HSP47 and sFlt-1 levels and the Gensini score.Results Compared with the control group,patients in the stable angina group had higher levels of hypertension,hyperlipidemia,total cholesterol,C-reactive protein,triglyceride,homocysteine,low-density lipoprotein cholesterol,HSP47,and sFlt-1,and lower high-density lipoprotein cholesterol level(t/χ2=6.024,4.628,2.388,5.380,2.125,5.682,2.169,7.972,9.747,2.460;P<0.05).The levels of HSP47 and sFlt-1 in mild stenosis group,moderate stenosis group and severe stenosis group were increased successively(P<0.05).The levels of HSP47 and sFlt-1 in single-vessel disease group,two-vessel disease group and three-vessel disease group were increased successively(P<0.05).HSP47 and sFlt-1 levels were positively correlated with the Gensini score(r=0.538,0.641;P<0.01).Multiple logistic regression analysis showed that HSP47 and sFlt-1 levels were independent influencing factors of the Gensini score(OR=3.971,3.508;P<0.01).Conclusion Serum levels of HSP47 and sFlt-1 in patients with stable angina are elevated and correlated with the severity of coronary artery disease.
