Correlation analysis between serum desmoglein 2 and galectin-3 mRNA expression levels and in-hospital sudden cardiac death in patients with acute myocardial infarction

Abstract
Objective To analyze the correlation between serum desmoglein 2(DSG2)and galectin-3(Gal-3)mRNA expression levels and in-hospital sudden cardiac death(SCD)in patients with acute myocardial infarction(AMI).Methods A total of 89 AMI patients who experienced in-hospital SCD at Handan Central Hospital from January 2019 to August 2022 were selected as the SCD group,and 92 hospitalized patients with pure AMI during the same period were selected as the pure AMI group.General clinical data of the two groups were compared.Serum DSG2 and Gal-3 mRNA expression levels were detected by real-time fluorescent quantitative PCR.Pearson method was used to analyze the correlations of serum DSG2 and Gal-3 mRNA expression levels with left ventricular ejection fraction(LVEF),cardiac troponin T(cTnT),and each other in AMI patients with in-hospital SCD.Logistic regression analysis was applied to analyze factors influencing in-hospital SCD in AMI patients.The predictive value of serum DSG2 and Gal-3 mRNA expression levels for in-hospital SCD in AMI patients was evaluated using receiver operating characteristic(ROC)curve analysis.Results Compared with the pure AMI group,the SCD group had lower LVEF and serum DSG2 mRNA expression levels,and higher cTnT and serum Gal-3 mRNA expression levels(t=6.163,8.288,16.484,10.009;P<0.05).In AMI patients with SCD,serum DSG2 mRNA expression levels were positively correlated with LVEF and negatively correlated with cTnT and Gal-3 mRNA levels(r=0.476,-0.379,-0.598;P<0.05).Gal-3 mRNA expression levels were negatively correlated with LVEF and positively correlated with cTnT(r=-0.362,0.428;P<0.05).The areas under the ROC curve(AUC)for predicting in-hospital SCD using serum DSG2 and Gal-3 mRNA levels were 0.869 and 0.854,respectively.The combined prediction of AUC for SCD in AMI patients was 0.946,with a sensitivity of 94.4%and specificity of 81.5%,which was significantly higher than using either marker alone(Z=2.453,2.778;P<0.05).Conclusion Serum DSG2 mRNA expression is low and Gal-3 mRNA expression is high in AMI patients with SCD.These biomarkers may provide predictive value for early prevention and management of SCD in AMI patients.关键词
