基于孕早期多维度指标的各年龄段妊娠糖尿病风险因素分析OA
Analysis of risk factors for gestational diabetes mellitus in all age groups based on multidimensional indicators in early pregnancy
目的 基于孕早期产检获得的病史、体检与实验室检测等多维临床信息,运用logistic回归分析探究孕早期妊娠糖尿病(GDM)风险因素,促进精准防控.方法 回顾性分析 2019 年 1 月至 2021 年 1 月厦门市妇幼保健院收治的 1205 例孕妇的孕早期临床信息,包括年龄、孕前体重身高、受教育程度、孕产次、腹围、谷丙转氨酶、平均动脉压(MAP)、GDM病史、糖尿病家族史等.结果 经孕 24~28 周口服葡萄糖耐量试验确诊为GDM有 280 例,患病率为 23.24%.经logistic回归分析发现,总体人群中高龄、孕前高体重指数(BMI)、GDM史和高值MAP是孕早期GDM的危险因素(P<0.05).<30 岁孕妇仅GDM病史与孕早期GDM显著相关;30~35 岁孕妇孕前高BMI、GDM病史和高值MAP是孕早期GDM的危险因素;>35 岁孕妇孕前高BMI、GDM病史和糖尿病家族史是孕早期GDM的危险因素(P<0.05).结论 不同年龄段孕早期的GDM风险因素分布特征具有差异性.在临床诊疗工作中,应开展精准防控、分层管理,以改善妊娠结局,促进母婴健康.
Objective Based on multidimensional clinical information of medical history,physical examination and laboratory tests obtained from prenatal checkups during early pregnancy,to explore the risk factors of gestational diabetes mellitus(GDM)in early pregnancy using logistic regression analysis and to promote precise prevention and control.Methods Clinical information of 1205 pregnant women in early pregnancy in Xiamen Maternal and Child Health Hospital from January 2019 to January 2021 was retrospectively analyzed,including age,pre-pregnancy weight and height,education level,gravidity and parity history,abdominal circumference,alanine aminotransferase,mean arterial pressure(MAP),history of GDM,and family history of diabetes mellitus.Results GDM was diagnosed by oral glucose tolerance test at 24-28 weeks of pregnancy in 280 cases with a prevalence of 23.24%.Logistic regression analysis revealed that advanced age,high pre-pregnancy body mass index(BMI),history of GDM and high MAP values were risk factors for GDM in early pregnancy in the overall population(P<0.05).Only history of GDM in pregnant women<30 years of age was significantly associated with GDM in early pregnancy.High pre-pregnancy BMI,history of GDM and high value of MAP in pregnant women 30-35 years of age were risk factors for GDM in early pregnancy.High pre-pregnancy BMI,history of GDM and family history of diabetes in pregnant women>35 years of age were risk factors for GDM in early pregnancy(P<0.05).Conclusion The distribution characteristics of risk factors for GDM in early pregnancy are different in various age groups.Precise prevention and control,and stratified management should be carried out in clinical practice to improve pregnancy outcomes and promote maternal and infant health.
