多模态磁共振成像在肝细胞肝癌通过微血管侵犯转移诊断中的应用研究
Application research of multi-modal magnetic resonance imaging in the diagnosis of hepatocellular carcinoma through microvascular invasion and metastasis

Abstract
Objective To explore the value of multi-modal magnetic resonance imaging in the assessment of hepatocellular carcinoma through microvascular invasion and metastasis.Methods A total of 60 patients with hepatocellular carcinoma who visited the Second Affiliated Hospital of Qiqihar Medical University from January 2022 to December 2023 were selected as the research subjects,with pathological results as the gold standard.The diagnostic value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI),diffusion weighted imaging(DWI),and multi-modal magnetic resonance imaging(DCE-MRI combined with DWI)in microvascular invasion and metastasis of hepatocellular carcinoma were compared and analyzed.Results Taking pathological results as the gold standard,the diagnostic sensitivity and specificity of multi-modal magnetic resonance imaging in 60 patients with hepatocellular carcinoma were higher than that of DCE-MRI,and the difference was statistically significant(χ2=4.336,4.247,P＜0.05).The diagnostic sensitivity and specificity of multi-modal magnetic resonance imaging were higher than those of DWI imaging,and the difference was statistically significant(χ2=5.263,6.844,P＜0.05).There was no significant difference in diagnostic sensitivity and specificity between DCE-MRI and DWI(χ2=0.051,0.419,P＞0.05).DCE-MRI(AUC=0.793),DWI(AUC=0.823)and DCE-MRI combined with DWI(AUC=0.898)had predictive value for microvascular invasion and metastasis of hepatocellular carcinoma(P＜0.05).Conclusion Multi-modal magnetic resonance imaging can significantly improve the diagnostic accuracy of microvascular invasion in hepatocellular carcinoma patients,effectively predict the risk of metastasis through microvascular invasion in HCC patients,and provide important assistance for the development of clinical treatment plans.关键词
