Abstract
Objective To assess the changes in intra-pancreatic fat deposition and the differences in pancreatic microcirculation in patients with different grades of glucose tolerance using magnetic resonance imaging(MRI),as well as their correlation.Methods A total of 77 patients with type 2 diabetes mellitus(T2DM)admitted to Hai'an People's Hospital from January 2022 to September 2023 were retrospectively selected,and 59 healthy individuals were selected as the control group.All participants underwent iterative decomposition of water and fat with echo asymmetry and least squares estimation(IDEAL-IQ)and dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI).Quantitative parameters[volume transfer constant(Ktrans),extravascular extracellular space volume fraction(Ve),fractional plasma volume(Vp)]were calculated.Clinical data were recorded,and the differences in intra-pancreatic fat deposition and pancreatic microcirculation were compared between T2DM patients and healthy controls.Spearman correlation analysis was used to examine the correlation between intra-pancreatic fat deposition and pancreatic microcirculation.The predictive efficacy of pancreatic head fat fraction,Ktrans,and Vp alone or in combination was assessed using receiver operating characteristic(ROC)curves.Results The body mass index(BMI),fat fraction of pancreatic head,body,and tail,and mean pancreatic fat fraction were all higher in the T2DM group compared to the control group(P<0.05).Statistical differences were found in Ktrans and Vp between the T2DM group and the control group(P<0.05),whereas no significant difference was observed in Ve between the two groups(P>0.05).Spearman correlation analysis showed that Ktrans was positively correlated with the fat fraction of the pancreatic head,body,and tail,and the mean pancreatic fat fraction vp was negatively(P<0.05),while Ve showed no correlation(P>0.05).The area under the curve(AUC)of separate tests of pancreatic head fat fraction,Ktrans,and Vp for T2DM was 0.834,0.716,and 0.828,respectively.The combined test of the three yielded the best overall predictive performance(AUC is 0.917),with a sensitivity and specificity of 90.9%and 84.7%,respectively.Conclusion MRI technology has certain advantages in assessing pancreatic microcirculatory changes and intra-pancreatic fat deposition in patients with T2DM.It can be used for clinical evaluation of the degree of pancreatic cell damage and provide imaging evidence for the occurrence and development prediction of T2DM.关键词
