外侧单髁膝关节置换术治疗膝外侧间室骨关节炎OA
Lateral unicondylar knee arthroplasty for treatment of lateral compartment osteoarthritis of knee
目的 探讨外侧单髁膝关节置换术(LUKA)治疗膝外侧间室骨关节炎的疗效.方法 采用LUKA治疗25 例膝外侧间室骨关节炎患者.记录手术情况、髋-膝-踝外翻角、膝关节屈曲活动度以及术后并发症发生情况.采用HSS评分评估膝关节功能.结果 患者均获得随访,时间 24~51 个月.切口长度 6~8(6.8±0.6)cm,术中出血量100~200(145.8±12.4)ml,手术时间69~140(110.0±9.7)min.术后髋-膝-踝外翻角、术后膝关节屈曲活动度、末次随访时膝关节功能 HSS 评分均较术前改善,差异均有统计学意义(P<0.05).术后均未发生感染、切口愈合不良等并发症,内侧间室退变及髌股关节炎均未见进展.末次随访时,X线片显示膝关节假体位置均良好,内固定牢靠,未见松动.结论 LUKA治疗膝外侧间室骨关节炎,术后患者膝关节屈伸功能及稳定性均良好,并发症少,创伤小.
Objective To investigate the efficacy of lateral unicondylar knee arthroplasty(LUKA)in the treatment of lateral compartment osteoarthritis of knee.Methods The 25 patients with lateral compartment osteoarthritis of knee were treated with LUKA.The surgery situation,valgus angle of the hip-knee-ankle knee flexion activity range and post-operative complications were recorded.HSS score was used to evaluate the knee joint function.Results All patients were followed up for 24~51 months.The incision length was 6~8(6.8±0.6)cm,the intraoperative blood loss was 100~200(145.8±12.4)ml,and the operative time was 69~140(110.0±9.7)min.The postoperative valgus an-gle of the hip-knee-ankle,the knee joint flexion motion range and the knee joint HSS score of the last follow-up were all improved,compared with those before surgery,and the differences were statistically significant(P<0.05).There were no complications such as infection and poor incision healing,and degeneration of the medial compartment and patellofemoral arthritis.At the last follow-up,X-ray radiographs showed that the knee prosthesis was in good position,the internal fixation was stable,and no loosening was observed.Conclusions LUKA in the treatment of lateral com-partment osteoarthritis of knee has the advantages of fast functional recovery,fewer complications and less trauma.
