三维数字化测量辅助全膝关节置换术治疗重度膝骨关节炎OA
Three-dimensional digital measurement assisted total knee arthroplasty in the treatment of se-vere knee osteoarthritis
目的 探讨三维数字化测量重度膝骨关节炎患者下肢相关角度在全膝关节置换术(TKA)中的应用效果.方法 将80 例重度膝骨关节炎患者根据随机数字表法分为X线测量组(采用传统TKA治疗,40 例)和三维数字化测量组(采用三维数字化测量辅助TKA治疗,40 例).比较两组术前测量的下肢相关角度的差异、手术情况、术后3 个月HSS评分及膝关节活动度.结果 两组均获得随访,时间3~6 个月.髋膝踝角、股胫角、胫骨角X线测量组均大于三维数字化测量组(P<0.05),胫骨平台后倾角、股骨角、股骨外翻角两组比较差异均无统计学意义(P>0.05).手术时间、术中出血量、术后引流量X线测量组均长(多)于三维数字化测量组(P<0.05).HSS评分两组术后3 个月均高于术前(P<0.05),术后 3 个月两组比较差异无统计学意义(P>0.05).膝关节屈曲活动度两组术后3 个月均大于术前(P<0.05),术后3 个月三维数字化测量组大于X线测量组(P<0.05).结论 与X线测量相比,三维数字化测量重度膝骨关节炎患者的下肢相关角度更准确,辅助TKA治疗手术时间更短,术中出血量以及术后引流量更少,术后膝关节活动度更满意.
Objective To investigate the application effect of three-dimensional(3D)digital measurement of lower limb related angle of severe knee osteoarthritis patients in total knee arthroplasty(TKA).Methods The 80 patients with severe knee osteoarthritis were divided into X-ray measurement group(traditional TKA treatment,40 cases)and 3D digital measurement group(3D digital measurement assissted TKA treatment,40 cases)according to random num-ber table method.The difference of the lower limb alignment related angle measured before surgery,operation situa-tion,HSS score at 3 months after surgery and knee activity were compared between the two groups.Results Both groups were followed up for 3~6 months.The hip-knee-ankle angle,femorotibial angle,tibial angle in X-ray measure-ment group were larger than those in 3D digital measurement group(P<0.05),and there were no statistical differ-ences in the posterior slope angle of tibial plateau,femur angle and femoral valgus angle between the two groups(P>0.05).Operation time,intraoperative blood loss and postoperative drainage volume were all longer(more)in X-ray measurement group than those in 3D digital measurement group(P<0.05).At 3 months after surgery,HSS scores:Two groups were higher that before surgery(P<0.05),and there was no statistical difference between the two groups(P>0.05).The knee flexion activity:At 3 months after surgery,both groups was higher than that before surgery(P<0.05),and 3D digital measurement group was larger than that in the X-ray measurement group(P<0.05).Conclusions Compared with X-ray measurement,3D digital measurement of lower extremity related angle in patients with severe knee osteoarthritis is more accurate,assisted TKA treatment has shorter operation time,less intraoperative blood loss and postoperative drainage volume,and more satisfactory postoperative knee activity.
