Abstract
Objective To investigate the application effect of tunnel control technique in closed reduction and intr-amedullary nail fixation for treatment of femoral subtrochanteric fractures.Methods The 20 patients with femoral subtrochanteric fractures were treated by closed reduction and intramedullary nail fixation using tunnel control tech-nique.The operation time,intraoperative blood loss,intraoperative fluoroscopy frequency,quality of fracture reduction,healing time and postoperative complications were recorded.Harris score was used to evaluate hip joint function.Re-sults All patients were followed up for 6~24 months.The operative time was 92~186(116.2±15.6)min,the in-traoperative blood loss was 125~650(180.5±70.4)ml,and the fluoroscopy frequency was 24~46(28.4±5.2)times.After operation,7 cases were anatomic reduction and 13 cases were functional reduction.All fractures healed within 6~18(7.8±3.2)months.All the incisions healed in one-stage.There were no complications such as deep vein thrombosis formation of lower limbs,nail withdrawl and internal fixation breakage,1 case had gastrointestinal hemorrhage and 2 cases had pulmonary infection,all of them were cured after symptomatic treatment.At the last fol-low-up,the Harris scores were 78~96(88.3±2.5)points,of which 12 cases were excellent,5 good and 3 fair,the excellent-good rate was 17/20.Conclusions For the treatment of femoral subtrochanteric fracture,closed reduction and intramedullary nail fixation with tunnel control technique can not only achieve satisfactory fracture reduction,but also reduce surgical trauma,protect the blood supply of the fractured end,and more conducive to the recovery of hip joint function.关键词
