关节镜辅助微创经皮钢板内固定治疗胫骨平台骨折的疗效
Efficacy of arthroscopy-assisted minimally invasive percutaneous plate osteosynthesis for treatment of tibial plateau fractures

Abstract
Objective To investigate the efficacy of arthroscopy-assisted minimally invasive percutaneous plate osteo-synthesis(MIPPO)in the treatment of tibial plateau fractures.Methods A total of 113 patients with tibial plateau fractures were divided into an observation group(treated with arthroscopy-assisted MIPPO,n=54)and a control group(treated with traditional open reduction and plate internal fixation,n=59).Perioperative indicators,fracture healing time,postoperative complications,pain VAS,knee flexion range of motion,and radiographic parameters(femorotibial angle,varus angle,posterior slope angle,knee joint space width)were compared between the two groups.Knee joint function was evaluated by using HSS score.Results All patients were followed up for 9 months.The observation group had shorter(less)incision length,intraoperative blood loss,postoperative drainage volume,time to ambulation,time to full weight-bearing,and fracture healing time,compared to the control group(P<0.01).The incidence of postoperative complications in the observation group was lower than that in the control group(P<0.05).At 3 months postoperatively,the pain VAS in the observation group was lower than that in the control group(P<0.05),and the excellent-good rate of knee joint function evaluated with HSS score was higher in the observation group than that in the control group(P<0.05).Femorotibial angles:Two groups had no statistical differences between the last follow-up and the preopration(P>0.05),and there was no statistical difference between the two groups(P>0.05).Varus angle,posterior slope angle and knee joint space width:At the last follow-up,both groups were smaller than those of the preo-peration(P<0.05),the data of observation group were all smaller than those of the control group(P<0.05).At the final follow-up,the knee flexion range of motion was greater in the observation group than that in the control group(P<0.05).Conclusions Arthroscopy-assisted MIPPO has the advantages of less trauma,less blood loss and fewer postoperative complications,less postoperative pain,and better fracture healing and knee function recovery.关键词
