摘要

Abstract

Objective To evaluate the influencing factors of adjacent vertebral fracture after percutaneous kyphoplasty(PKP)for osteoporotic vertebral compression fracture(OVCF)using finite element analysis.Methods A finite ele-ment model of osteoporotic T10~L3 vertebrae was established in a healthy young male volunteer with 64-slice spiral CT scanning.The osteoporotic model of T10~L3 vertebrae was set as control group,and the osteoporotic model of L2 vertebrae was set as observation group.The von Mises stress on the T11~L3 related structures was observed by apply-ing 1 200 N vertical downward force on the top of the T10 vertebral body.Results ① Control group:compared with other vertebrae,the von Mises stress of cortical bone,cancellous bone and upper and lower endplates of T12 vertebrae was higher(P<0.05),the maximum von Mises stress of cortical bone and upper and lower endplates were larger than those of cancellous bone(P<0.05).② Observation group:after PKP treatment,the biomechanical characteristics of the spine changed because of the effect of bone cement.The maximum von Mises stress in the vertebrae was trans-ferred from T12 to L1,the stress of the adjacent vertebral endplates was strengthened,with bone cement increased sig-nificantly.Conclusions ① When the spine is subjected to greater external forces,the T12 vertebrae is more prone to fracture,which is related to its location in the most central segment of the thoracolumbar segment,and the influence of physiological curvature is dominant.②After the treatment of OVCF with PKP,the physiological curvature and the bio-mechanical changes of the bone cement to the adjacent vertebrae are coexisted,and the biomechanical changes of the bone cement to the adjacent vertebrae are the main factors.
