两种腰椎融合术治疗单节段腰椎退行性疾病的疗效及成本-效果比较
Comparison of the efficacy and cost-effectiveness between two types of lumbar interbody fu-sion for treatment of single-segment lumbar degenerative diseases

Abstract
Objective To compare the clinical efficacy and cost-effectiveness of minimally invasive transforaminal lumbar interbody fusion(MIS-TLIF)and open transforaminal lumbar interbody fusion(TLIF)in the treatment of sin-gle-level lumbar degenerative disease.Methods The 70 patients with single-level lumbar degenerative diseases were divided into MIS-TLIF group(35 patients were treated with MIS-TLIF)and TLIF group(35 patients were treated with open TLIF),according to different surgical methods.The surgical status,clinical efficacy,interbody fusion rate at 12 months after surgery,cost-effectiveness ratio(CER)and incremental cost-effectiveness ratio(ICER)were com-pared between the two groups.Results All patients were followed up for 12 months.The intraoperative blood loss vol-ume and the removal time of drainage tube postoperatively were less(earlier)in MIS-TLIF group than those in TLIF group(P<0.05).The operation time of MIS-TLIF was longer than that of TLIF group(P<0.05).At 3 and 12 months after surgery,pain VAS and ODI of two groups were improved than those before surgery(P<0.05),there was no statistical difference in pain VAS between the two groups(P>0.05),and ODI of MIS-TLIF group was lower than that of TLLF group(P<0.05).The rate of interbody bone graft fusion at 12 months after surgery in TLIF group was slightly lower than that in MIS-TLIF group,but the difference had no statistical significance(P>0.05).The CER value:the MIS-TLIF group was 373.9,TLIF group was 433.3,the MIS-TLIF group was smaller than the TLIF group,and the ICER value was-547.01.Conclusions Both MIS-TLIF and TLIF can achieve good clinical efficacy in the treatment of single-level lumbar degenerative diseases.MIS-TLIF is a kind of more cost-effective surgical option.关键词
