围术期小剂量艾司氯胺酮对产妇剖宫产术后产后抑郁的影响
Effect of low dose esketamine in perioperative period on postpartum depression after cesarean section
摘要
Abstract
[Objective]To investigate the effect of low dose esketamine on postpartum depression after cesarean section.[Methods]Eighty full-term single parturients who underwent cesarean section under combined lumbar and epidural anesthesia were randomly divided into esketamine group and control group,with 40 cases in each group.Five minutes after the umbilical rupture of the fetus,esketamine group was given 0.25 mg/kg esketamine+0.9%normal saline to 10 mL pump for 10 min.The control group was injected with 0.9%normal saline 10 mL pump for 10min.The postoperative analgesia regimen consisted of sufentanil 2 μg/kg+0.9%normal saline,a total of 100 mL,continuous infusion at the rate of 2 mL/h,the infusion time was 48 hours,the maintenance dose was 2 mL/h,the patient controlled analgesia(PCA)was 1 mL/time,and the locking time was 15 minutes.If the Visual Analogue Scale(VAS)score is greater than or equal to 7 points,flurbiphenolate should be used for remediation.[Results]Compared with the control group,the incidence of postpartum depression at 7 days after surgery in esketamine group was significantly reduced(P<0.05),and the incidence of postpartum depression at 42 days after surgery had no significant difference(P>0.05).Compared with the control group,the pain score of esketamine group was lower 24 hours after operation(P<0.05).Compared with the control group,the analgesic satisfaction of esketamine group was higher(P<0.05),and the incidence of nausea,vomiting and dizziness was not significantly different between the two groups(P>0.05).[Conclusion]A single intravenous injection of 0.25 mg/kg esketamine during perioperative period has a significant effect on postpartum depression after cesarean section,and also provides a better analgesic experience for parturients.关键词
