DWI结合SWI对急性脑梗死患者脑微出血的诊断价值

Abstract
Objective:To explore the diagnostic value of diffusion weighted imaging(DWI)combined with susceptibility weighted imaging(SWI)for cerebral microbleeds in patients with acute cerebral infarction.Method:The study included 102 patients with acute cerebral infarction admitted to Huai'an No.82 Hospital from January 2021 to January 2024.All patients underwent DWI and SWI examination.Taking pathological diagnosis results as the gold standard,the diagnostic value of DWI and SWI alone and combined examination in patients with acute cerebral infarction were compared.Result:Among 102 patients,95 cases were complicated with cerebral microbleeds.There were 67 cases of cerebral microhemorrhage diagnosed by DWI,78 cases of cerebral microhemorrhage diagnosed by SWI,and 87 cases of cerebral microhemorrhage diagnosed by combined diagnosis.The accuracy,sensitivity,specificity,positive predictive value and negative predictive value of DWI and SWI combined in the diagnosis of cerebral microhemorrhage in patients with acute cerebral infarction were 91.18%,91.58%,85.71%,98.86%and 42.86%,respectively.The accuracy of combined diagnosis of cerebral microbleeds was higher than that of single detection,and the sensitivity and negative predictive value were higher than that of DWI alone detection(P<0.05).Conclusion:The combined use of DWI and SWI can improve the sensitivity and specificity of diagnosing cerebral microbleeds in patients with acute cerebral infarction.关键词
