摘要

Abstract

Objective:To investigate the effects of miR-27a and intestinal flora changes on gestational diabetes mellitus.Method:A total of 100 patients with gestational diabetes mellitus admitted to Guangzhou Conghua District Maternal and Child Health Hospital from January 2021 to December 2023 were selected as the observation group,and another 100 healthy pregnant women in the same period were selected as the control group.The levels of miR-27a,intestinal flora and glucose and lipid metabolism were compared between the observation group and the control group.According to the pregnancy outcome,the observation group was divided into normal delivery group and adverse pregnancy group.The miR-27a,intestinal flora level and glucose and lipid metabolism levels of the two groups were compared.The influencing factors of pregnancy outcome were analyzed by logistic regression.Receiver operating characteristic(ROC)curve was used to analyze the value of miR-27a and gut microbiota levels in evaluating adverse pregnancy outcomes.Result:The levels of miR-27a,Bifidobacterium and Lactobacillus in observation group were lower than those in control group(P<0.05),while the levels of Enterobacterium and Enterococcus in observation group were higher than those in control group(P<0.05).Fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),low density lipoprotein cholesterol(LDL-C)in observation group were higher than those in control group(P<0.05),high density lipoprotein cholesterol(HDL-C)was lower than that in control group(P<0.05).The levels of miR-27a,Bifidobacterium and Lactobacillus in the adverse pregnancy group were lower than those in the normal delivery group(P<0.05),and the levels of Enterobacter and Enterococcus were higher than those in the normal delivery group(P<0.05).logistic regression analysis showed that miR-27a,Lactobacillus and Bifidobacterium were all protective factors for pregnancy outcome in patients with gestational diabetes mellitus,while Enterobacter,Enterococcus were independent risk factor for pregnancy outcome in patients with gestational diabetes mellitus(P<0.05).The AUC of the combined detection of miR-27a,Enterobacter,Lactobacillus,Enterococcus,and Bifidobacterium in evaluating adverse pregnancy outcomes was higher than that of the single detection(P<0.05).Conclusion:Patients with gestational diabetes mellitus mostly have intestinal flora and glucose and lipid metabolism disorders,and the expression of miR-27a is low.miR-27a and intestinal flora have certain value in evaluating adverse pregnancy outcomes in patients with gestational diabetes mellitus.
