柴胡类方治疗伴焦虑抑郁的干眼患者的临床疗效观察
Clinical Efficacy Observation of Bupleurum-based Formulas in the Treatment of Dry Eye Patients with Anxiety and Depression

Abstract
OBJECTIVE To observe the clinical efficacy of Bupleurum-based formulas in the treatment of dry eye patients with anxiety and depression.METHODS A total of 116 patients(116 eyes)with dry eye who visited the outpatient department of the Department of Ophthalmology,the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from November 2022 to October 2023 were enrolled and randomly divided into a Chinese medicine group(CMG,39 patients,39 eyes),a western medicine group(WMG,39 patients,39 eyes),and a combined group(CG,38 patients,38 eyes).The Chinese medicine group was treated with Bupleurum-based Chinese medicine decoctions,the western medicine group was treated with 0.1%sodium hyaluronate eye drops,and the combined group was treated with Bupleurum-based formulas combined with sodium hyaluronate eye drops.All three groups were treated for four weeks.Before and after treatment,the Schirmer I test(SⅠT),tear film break-up time(BUT),corneal fluorescein staining(FL),self-rating anxiety scale(SAS),self-rating depression scale(SDS),and hospital anxiety and depression scale(HADS)scores of the subjects were measured,and the data were recorded and statistically analyzed.RESULTS(1)Dry eye-related indicators:Compared before and after treatment,the SⅠT of the three groups after treatment(tCMG=5.601,tWMG=6.692,tCG=8.658,all P=0.000)and BUT(tCMG=5.074,tWMG=6.586,tCG=13.761,all P=0.000)were both longer than those before treatment.The FL scores of the western medicine group and the combined group after treatment(tWMG=-2.387,P=0.019;tCG=-8.535,P=0.000)were both lower than those before treatment,and the differences were statistically significant.There were no significant differences in the FL scores of the Chinese medicine group after treatment compared with that before treatment(P>0.05).After treatment,there were statistically significant differences in SⅠT,BUT,and FL among the three groups(FSⅠT=11.426,FBUT=13.726,both P=0.000;FFL=5.757,P=0.004).After treatment,pairwise comparisons among the groups showed that the SⅠT of the combined group after treatment was longer than that of the Chinese medicine group and the western medicine group(tCMG=4.485,P=0.000;tWMG=3.042,P=0.003),and the FL scores were lower than those of the Chinese medicine group and the western medicine group(tCMG=-3.785,P=0.000;tWMG=-2.505,P=0.014).After treatment,both the BUT of the western medicine group and the combined group were longer than that of the Chinese medicine group(tWMG=2.407,P=0.019;tCG=4.987,P=0.000),and the BUT of the combined group was longer than that of the western medicine group(t=3.001,P=0.004),and the differences were statistically significant.There were no statistically significant differences in the SⅠT and FL scores of the western medicine group after treatment compared with those of the Chinese medicine group(P>0.05).(2)Anxiety and depression-related indicators:Compared before and after treatment,the SAS scores(tCMG=-37.449,tCG=-25.381,both P=0.000),SDS scores(tCMG=-14.311,tCG=-21.886,both P=0.000),and HADS scores(tCMG=-21.272,tCG=-17.495,both P=0.000)of the Chinese medicine group and the combined group after treatment were all lower than those before treatment,and the differences were statistically significant.There were no statistically significant differences in the SAS,SDS,and HADS scores of the western medicine group after treatment compared with those before treatment(P>0.05).After treatment,there were statistically significant differences in the SAS,SDS,and HADS scores among the three groups(FSAS=106.440,FSDS=102.603,FHADS=32.544,all P=0.000).After treatment,pairwise comparisons among the groups showed that the SAS,SDS,and HADS scores of the western medicine group after treatment were all higher than those of the Chinese medicine group,and the differences were statistically significant(tSAS=11.732,tSDS=11.909,tHADS=5.523,all P=0.000).The SAS,SDS,and HADS scores of the combined group after treatment were all lower than those of the western medicine group,and the differences were statistically significant(tSAS=-12.820,tSDS=-12.728,tHADS=-7.257,all P=0.000).There were no statistically significant differences in the SAS,SDS,and HADS scores of the combined group after treatment compared with those of the Chinese medicine group(P>0.05).(3)Clinical efficacy:There was a statistically significant difference in the total effective rate among the three groups after treatment(χ2=7.256,P=0.027).The total effective rate of the combined group was higher than that of the Chinese medicine group and the western medicine group,and the differences were statistically significant(χ2CMG=5.692,P=0.015;χ2WMG=7.120,P=0.008).There was no statistically significant difference in the total effective rate of the western medicine group compared with that of the Chinese medicine group(P>0.05).CONCLUSIONS Bupleurum-based formulas combined with 0.1%sodium hyaluronate eye drops can increase the tear secretion volume,prolong the BUT,and improve the symptoms of anxiety and depression.The efficacy is better than that of single Bupleurum-based formulas and single western medicine.关键词
