雷火灸联合养阴润目方治疗干眼的临床疗效评价
Clinical Efficacy Evaluation of Thunder-fire Moxibustion Combined with Yangyin Runmu Formula in Treating Dry Eye

Abstract
OBJECTIVE To observe the clinical efficacy of Thunder-Fire Moxibustion combined with Yangyin Runmu Formula in treating dry eye syndrome.METHODS A total of 97 dry eye patients(153 eyes)treated at Zhangjiakou Fourth Hospital from May 2022 to May 2023 were enrolled and randomly divided into a Traditional Chinese Medicine(TCM)group(48 cases,75 eyes)and a combined group(49 cases,78 eyes).The TCM group received Yangyin Runmu Formula treatment,while the combined group received Thunder-Fire Moxibustion combined with Yangyin Runmu Formula treatment,with an average treatment duration of one month.Before and after treatment,the TCM symptom scores,ocular surface disease index(OSDI)scores,schirmer Ⅰ test(SⅠT),tear film breakup time(BUT),tear meniscus area(TTMA),visual function,and tear related factors[lysozyme,matrix metalloproteinase-9(MMP-9),tumor necrosis factor-α(TNF-α),interleukin-10(IL-10)]were measured and compared.RESULTS(1)TCM symptom scores:Both groups showed a significant reduction in TCM symptom scores after treatment(tTCM=23.668,tCG=28.928,both P=0.000).The combined group had a significantly lower TCM symptom score compared to the TCM group(t=12.467,P=0.000).(2)OSDI scores:Both groups showed a significant reduction in OSDI scores after treatment(tTCM=7.855,tCG=12.431,both P=0.000).The combined group had a significantly lower OSDI score compared to the TCM group(t=6.816,P=0.000).(3)SⅠT,BUT,and TTMA:Both groups showed significant improvements in SⅠT,BUT,and TTMA levels after treatment(TCM group:tSⅠT=23.164、t BUT=26.683、t TTMA=191.950,all P=0.000;combined group:tSⅠT=28.500、t BUT=30.892、t TTMA=178.337,all P=0.000.The combined group had significantly higher SⅠT,BUT,and TTMA levels compared to the TCM group(tSⅠT=9.014、tBUT=9.804、tTTMA=17.0554,all P=0.000).(4)Visual contrast sensitivity:Both groups showed significant improvements in visual contrast sensitivity at 12,18,and 24 c/d(TCM group:t12 c/d=11.135、t18 c/d=7.895、t24 c/d=9.119,all P=0.000;combined group:t12 c/d=18.023、t18 c/d=12.665、t24 c/d=12.006,all P=0.000).The combined group had significantly higher visual contrast sensitivity at 12,18,and 24 c/d compared to the TCM group(t12 c/d=7.710、t18 c/d=6.053,both P=0.000;t24 c/d=3.515,P=0.006).(5)Tear related factors:Both groups showed significant reductions in MMP-9 and TNF-α levels and significant increases in lysozyme and IL-10 levels after treatment(TCM group:tMMP-9=195.773、tTNF-α=13.158,tlysozym=4.219、tIL-10=6.538,all P=0.000;combined group:tMMP-9=234.974、tTNF-α=24.174,tlysozyme=10.132、tIL-10=13.712,all P=0.000).The combined group had significantly lower MMP-9 and TNF-α levels and significantly higher lysozyme and IL-10 levels compared to the TCM group(tMMP-9=10.109、tTNF-α=10.762,tlysozyme=5.471、tIL-10=5.621,all P=0.000).(6)Clinical efficacy:The total effective rate in the combined group was significantly higher than that in the TCM group(χ2=4.162,P=0.041).CONCLUSIONS Thunder-fire moxibustion combined with Yangyin Runmu Formula can reduce levels of inflammatory factors,enhance anti-inflammatory factors,tear dynamics,and tear secretion,prolong tear film breakup time,improve visual function,and enhance clinical efficacy.关键词
