加速康复外科在上尿路结石体系应用中的价值
The Value of Enhanced Recovery after Surgery in the Application of Upper Urinary Tract Calculi System

Abstract
Objective:To evaluate the value of enhanced recovery after surgery(ERAS)in the application of upper urinary tract calculi.Method:A retrospective study was conducted on 100 patients with upper urinary tract stones who were admitted to the Urology Department of Xiangya Changde Hospital from January 2023 to April 2024.All patients were divided into an experimental group and a control group according to different treatment methods,with 50 patients in each group.The experimental group constructed ERAS special treatment system for upper urinary tract calculi,while the control group received routine operation to treat upper urinary tract calculi.The incidence of complications,the first feeding time,the first exhaust time,hospital stay and various inflammatory indexes[including procalcitonin(PCT),interleukin-6(IL-6),C reactive protein(CRP)and tumor necrosis factor-α(TNF-α)]were compared between the two groups.Result:The incidence of postoperative complications in the experimental group(4％)was lower than that in the control group(16％),the difference was statistically significant(P<0.05).The first time of eating and exhalation in the experimental group after surgery were(7.46±2.19)and(14.27±4.16)hours,which were earlier than(11.58±2.64)and(27.06±3.53)hours in the control group,the hospitalization time in the experimental group was(7.94±1.73)days,which was shorter than(11.45±1.17)days in the control group,the differences were statistically significant(P<0.05).The postoperative inflammatory response indicators PCT,IL-6,CRP and TNF-α in the experimental group were(11.28±1.12)ng/L,(66.27±8.48)pg/mL,(14.48±2.94)mg/L,and(2.05±0.41)μg/L,respectively,which were lower than(14.03±1.62)ng/L,(70.51±13.21)pg/mL,(21.42±3.31)mg/L and(3.13±0.29)μg/L in the control group,the differences were statistically significant(P<0.05).Conclusion:The establishment of ERAS treatment system for patients with upper urinary calculi can significantly reduce the incidence of complications,eat and exhaust as early as possible,shorten the length of hospital stay,and there is little effect on inflammatory indicators compared with the conventional surgical treatment of upper urinary calculi.关键词
