Abstract
Objective:To investigate the relationship between inflammatory factors and disease severity and prognosis of patients with diabetic foot ulcer.Method:The clinical data of 98 patients with diabetic foot ulcer admitted to Taizhou Hospital of TCM from April 2021 to April 2024 were retrospectively analyzed,and the patients were divided into good prognosis group(n=67)and poor prognosis group(n=31)according to the prognosis.The levels of inflammatory factors[C reactive protein(CRP),white blood cell count(WBC),interleukin-6(IL-6),procalcitonin(PCT)]were compared in patients with diabetic foot ulcer with different infection degrees,Wagner grade and ischemic degrees.Multivariate logistic regression analysis was used to explore the factors affecting the poor prognosis of patients with diabetic foot ulcer,and the operative characteristic curve of subjects was constructed to evaluate the efficacy of age,inflammatory factors and ankle brachial index in predicting the poor prognosis of patients with diabetic foot ulcer.Result:The levels of CRP,WBC,IL-6 and PCT increased with the severity of infection degree and ischemia degree and the increase of Wagner grade,the differences were statistically significant(P<0.05).The age of the poor prognosis group was older than that of the good prognosis group,the levels of CRP,WBC,IL-6 and PCT were higher than those of the good prognosis group,and the ankle brachial index was lower than that of the good prognosis group,the differences were statistically significant(P<0.05).Multivariate logistic regression analysis showed that age,CRP,WBC,IL-6 and PCT were all risk factors for the poor prognosis of patients with diabetic foot ulcer,and the ankle brachial index was a protective factor for the poor prognosis of patients with diabetic foot ulcer(P<0.05).The combined determination of age,CRP,WBC,IL-6,PCT and ankle brachial index had the highest predictive value for the poor prognosis of patients with diabetic foot ulcer,with a sensitivity of 96.8%,a specificity of 92.5%,and an area under the curve(AUC)of 0.966.Conclusion:CRP,WBC,IL-6,PCT and ankle brachial index are closely related to the disease severity and prognosis of patients with diabetic foot ulcer,and the combined detection of the above indicators and age has high predictive value for the poor prognosis of patients.关键词
