摘要
Abstract
Objective:To investigate the effect of different doses of Esketamine in patients undergoing laparoscopic hysterectomy(LTH).Method:A total of 100 patients undergoing LTH in Xiangtan First People's Hospital from January 2021 to October 2023 were selected and divided into group A,group B and group C by lottery method.Patients in group A(33 cases)were given Esketamine 0.25 mg/kg,and patients in group B(33 cases)were given Esketamine 0.5 mg/kg,group C(34 cases)was given 0.9%Sodium Chloride Solution. The perioperative psychological changes[self-rating anxiety scale(SAS)and self-rating depression scale(SDS)],postoperative pain[numerical rating scale(NRS)]changes at 2 h,12 h and 24 h,and the incidence of adverse reactions after medication were compared among the three groups.Result:Compared with before surgery,SAS scores and SDS scores of patients in group A and group B were decreased after surgery, and the differences were statistically significant(P<0.05).At the same time, the scores of group A and group B were lower than those of group C,and the scores of group A were lower than those of group B,the differences were statistically significant(P<0.05).At 12 h and 24 h after surgery, the pain scores of the three groups were lower than those at 2 h after surgery, the differences were statistically significant(P<0.05).At 24 h after surgery, the pain scores of the three groups were lower than 12 h after surgery, and the difference was statistically significant(P<0.05).At 2 h,12 h and 24 h after surgery, the pain score of group C was higher than that of group A and group B,that of group A were higher than that of group B,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of restlessness, headache and gastrointestinal reaction among the three groups(P>0.05).The incidence of remedial analgesia in group A and group B was lower than that in group C,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of remedial analgesia between group A and group B(P>0.05).Conclusion:Different doses of Esketamine in LTH patients can improve the adverse mood of patients, relieve postoperative pain, and reduce the incidence of adverse reactions. However,in terms of improving patient psychology, low dose treatment is more effective, higher doses have lower pain scores.关键词
