Abstract
Objective:To explore the diagnosis value of combined detection of alpha-fetoprotein(AFP),alanine aminotransferase(ALT),total bile acid(TBA),and gamma-glutamyl transferase(GGT)in patients with cirrhosis.Method:A total of 120 patients with cirrhosis admitted to Nanjing Gaochun People's Hospital from January 2020 to December 2023 were included in the observation group,another 100 patients with hepatitis were included in the hepatitis group during the same period,and 100 healthy physical examination personnel were included in the control group.Serum levels of AFP,ALT,TBA and GGT were detected in all three groups.The levels of AFP,ALT,TBA and GGT were compared among the three groups and the patients with different grades of Child-pugh liver cirrhosis in observation group.The relationship between the levels of AFP,ALT,TBA,GGT and Child-Pugh grade of liver function was analyzed.The diagnosis value of combined detection of AFP,ALT,TBA and GGT in patients with cirrhosis was analyzed.Result:The levels of AFP,ALT,TBA and GGT in the three groups were gradually increased,and the levels of AFP,ALT,TBA and GGT in the observation group were higher than those in the hepatitis group and the control group,while the levels of AFP,ALT,TBA and GGT in the hepatitis group were higher than those in the control group,the differences were statistically significant(P<0.05).The levels of AFP,ALT,TBA and GGT in grade A,grade B and grade C patients were gradually increased,the levels of AFP,ALT,TBA and GGT in grade A patients were lower than those in grade B and grade C patients,and the levels of AFP,ALT,TBA and GGT in grade B patients were lower than those in grade C patients,the differences were statistically significant(P<0.05).Correlation analysis showed that serum levels of AFP,ALT,TBA and GGT were positively correlated with Child-Pugh classification of liver function(P<0.05).The receiver operating characteristic(ROC)curve analysis results showed that the sensitivity and specificity of the combined detection of serum AFP,ALT,TBA,and GGT levels for diagnosis liver cirrhosis were 93.3%and 99.0%respectively.The area under the ROC curve(AUC)was 0.923,which was better than detecting them separately.Conclusion:Serum AFP,ALT,TBA and GGT levels can effectively diagnosis the severity of cirrhosis patients,and their combined detection accuracy is higher than that of any single index detection,which is of great value for clinical diagnosis and efficacy evaluation.关键词
