王道坤教授衷中参西辨治慢性萎缩性胃炎思路探析
An analysis of professor WANG Daokun's thoughts on distinguishing and treating chronic atrophic gastritis by Chinese medicine with reference to Western medicine

Abstract
This article summarizes and analyzes professor WANG Daokun's thoughts on diagnosing and treating chronic atrophic gastritis(CAG)by Chinese medicine with reference to Western medicine.Professor WANG clearly classifies CAG as a type of"stomach stuffiness"syndrome.Due to the lack of specific clinical manifestations of this disease,he emphasizes that in clinical diagnosis,traditional TCM four-diagnosis methods should be combined with technological examination methods of Western medicine,advocating Chinese medicine with reference to Western medicine.Through observation,auscultation and olfaction,inquiry,and palpation,the etiology,pathogenesis,and syn-drome differentiation types of CAG can be determined.In combination with gastroscopy and pathological examina-tions,the lesions of the gastric mucosa and their severity can be visually understood.Professor WANG believes that the dysfunction of the gastric mucosal function in CAG belongs to the category of abnormal ascending and descen-ding functions of the spleen and stomach in TCM.The core pathogenesis is the failure of the clear yang of the spleen to ascend and the turbid yin of the stomach to descend,with a combination of deficiency and excess.In clinical treat-ment,both strengthening the body and eliminating pathogenic factors should be considered.Specialized prescriptions for specific diseases,such as the"Zhiwei Fangbian Capsule",should be given.The etiology and pathogenesis of this disease are complex,and it is most closely related to the liver,spleen and kidney.Phlegm-dampness,qi stagnation,blood stasis,and food retention are its main pathological factors,and there are various clinical syndrome types.Treat-ment should be based on the syndrome and prescription,not being limited to certain prescriptions or medicines.For severe cases,both treating the root cause and symptoms,and using both elimination and tonification methods are ap-propriate.On the basis of syndrome differentiation and medication,combinations of herbs such as Hedyotis diffusa,Agrimonia pilosa,and Patrinia scabiosaefolia are used to nourish qi and yin,remove blood stasis and detoxify to pre-vent canceration.Professor WANG's thoughts on diagnosing and treating CAG by Chinese medicine with reference to Western medicine has positive referential significance for contemporary clinical practice of TCM.TCM should strengthen the thinking and research on the"physical form";the communication and integration of"qi transforma-tion"and"physical form"in the theoretical system of TCM may be the basis and key for the development of TCM and even the integration of TCM and Western medicine.关键词
