牙周炎超重肥胖人群免疫炎症特点及与内脏脂肪关系研究
Characteristics of immune inflammation in overweight and obese people with periodontitis and its relationship with visceral fat area

Abstract
Objective:To investigate the characteristics of immune inflammation in overweight and obese people with periodontitis and its relationship with visceral fat area(VFA).Methods:A total of 206 patients with periodontitis admitted to our hospital from January 2022 to January 2024 were selected as the study subjects,and the patients were divided into normal group(BMI<25 kg/m2,n=69),overweight group(25 kg/m2≤BMI≤30 kg/m2,n=80),and obesity group(BMI>30 kg/m2,n=57).Immune inflammatory factors were measured using immune rate scattering turbidimetry,flow cytometry,and enzyme-linked immunosorbent assay.VFA was measured by DBA-550 bioelectrical impedance measurement.Logistic regression was used to analyze the relationship between immune inflammatory factors and VFA.Logistic regression model was used to analyze the interaction of risk factors.Results:There were significant differences in immunoglobulin G(IgG),IgA,interleukin-4(IL-4),IL-17,and VFA among the three groups of patients(P<0.05).The levels of IgG,IgA,IL-4,IL-17,CD4+,CD4+/CD8+and VFA in the obesity group were higher than those in the normal group and overweight group(P<0.05),and the lev-els of CD8+and IL-10 were lower than those in the normal group and overweight group(P<0.05).Logistic regression analysis showed that an increase in VFA was independently correlated with IgA and IL-4(P<0.05),but not with IgG and IL-17(P>0.05).There were significant differences in IgG,IgA,IgM,CD4+,CD8+,CD4+/CD8+,IL-4,IL-10,IL-17,and VFA levels be-tween the periodontitis group and the non periodontitis group(P<0.05).Stratified regression analysis showed that IL-4 and VFA had a significant negative impact on periodontitis(P<0.05).The additive interaction model showed significant additive interaction between IL-4 and VFA in the pathogenesis of periodontitis(P<0.05).Conclusion:There were significant differ-ences in the levels of immune inflammatory factors and VFA in periodontitis patients in the obesity group,and the risk of peri-odontitis was greatly increased when high IL-4 levels and overweight were present.关键词
