1990-2021年中国生殖道沙眼衣原体感染的疾病负担趋势
The disease burden of genital Chlamydia trachomatis infection in China from 1990 to 2021

Abstract
Objective To clarify the burden of genital Chlamydia trachomatis(CT)infection in China and its temporal trend.Methods Using data from the Global Burden of Diseases,Inju-ries,and Risk Factors Study(GBD)2021,we assessed the incidence,prevalence,and disabili-ty-adjusted life years(DALYs)of genital CT infection in 2021 in national-and provincial-level.The joinpoint regression was used to analyze the temporal trends of the above three types of bur-dens associated with disease from 1990 to 2021.Additionally,the GDP per capita as a cubic spline into a generalized linear model was used to explore the influence of economic status on the incidence,prevalence,and DALYs rates of age-standardized genital CT infection.Results In 2021,there were a total of 62.67 million new cases of genital CT infection in China,with an inci-dence rate of 4 405.16 per 100 000 population(age-standardized:4 163.33 per 100 000).The cases were around 38.74 million,with a prevalence of 2 722.86 per 100 000 population(age-standardized:2 563.92 per 100 000).The disability-adjusted life years(DALYs)due to genital CT infection were 25 980 person-years,with a DALYs rate of 1.83 per 100 000(age-standard-ized:1.70 per 100 000).The incidence rates and prevalence were stable,while DALYs had a declining trend[average annual percent change(AAPC):-0.32％,(95％CI:-0.45％～-0.19％)]between 1990 and 2021.Incidence rates and DALYs were higher in males than in females,whereas prevalence was higher in females than in males in 2021.From 1990 to 2021,the disease burden was decreased in males,while incidence rates and prevalence were increased in females,but DALYs were decreased.In 2021,the overall disease burden peaked at ages of 30-34.Except for the peak of DALYs in females at age 40-44 years,the disease burden peaked in age ranges consistent with the general population in both genders.The age-standardized inci-dence rates were the highest in western provinces and lowest in central provinces.From 1990 to 2021,Shanxi showed the fastest growth in age-standardized incidence rates and prevalence.The growth in age-standardized DALYs rate by province was not significant.From 2020 to 2021,GDP per capita showed a U-shaped association with age-standardized incidence rates and DALYs.Con-clusions Although the disease burden of genital CT infection emerges a declining trend,its inci-dence rates and prevalence remain high in China.Future prevention and control strategies should pay more attention to individuals aged 25-34 years and those residing in regions with either ex-tremely low or high economic status.关键词
