Abstract
Objective:To compare the efficacy of proximal femoral nail antirotation(PFNA)and artificial femoral head replacement(AFHR)in the treatment of senile unstable intertrochanteric femoral fractures.Methods:Data of 60 elderly patients with unstable intertrochanteric femoral fractures who received surgical treatment in Junan Hospital of Traditional Chinese Medicine from January 2023 to January 2024 were retrospective analyzed,and were divided into a control group and a study group according to different surgical methods,with 30 cases in each group.The control group received AFHR treatment and the study group received PFNA treatment.The indexes of operation and postoperative rehabilitation,stress response levels,hip function and occurrence of complications were compared between the two groups.Results:The intraoperative blood loss of the study group was less than that of the control group,and the operation time,the first time of getting out of bed after surgery and the length of hospital stay were shorter than those of the control group,the differences were statistically significant(P<0.05).After operation,the levels of cortisol,epinephrine and norepinephrine in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).At 1,3 and 6 months after operation,the Harris Hip Scores of the study group were higher than those of the control group,the differences were statistically significant(P<0.05).The incidence of complications in the study group was 6.67%,which was lower than 26.67%in the control group,and the difference was statistically significant(P<0.05).Conclusion:Compared with AFHR,PFNA treatment in elderly patients with unstable intertrochanteric femoral fractures shows certain advantage in terms of surgical outcome,postoperative rehabilitation,stress response control,hip function recovery and complication management.关键词
