Abstract
Objective:To compare the clinical effect of intramedullary locking nail fixation(ILN)and plate screw fixation(PSF)in the treatment of bone nonunion after traumatic fracture of extremities.Methods:Data of 90 patients with bone nonunion after traumatic fracture of extremities treated in Shanghe Traditional Chinese Medicine Hospital from January 2023 to January 2024 were retrospectively analyzed.According to different fracture fixation methods,they were divided into the control group(n=46)and the study group(n=44).The control group was treated with PSF,while the study group was treated with ILN.The surgical indexes,stress levels,bone metabolism levels and complications were compared between the two groups.Results:The intraoperative blood loss of the study group was less than that of the control group,and the operation time,postoperative getting out of bed time,hospital stay time and fracture healing time were shorter than those of the control group,the differences were statistically significant(P<0.05).After surgery,the levels of cortisol,epinephrine and norepinephrine in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).After surgery,the levels of N-terminal in the middle osteocalcin,β collagen degradation products,procollagen type Ⅰ C-terminal peptide and bone gamma-hydroxyglutamate protein in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).The complication rate of the study group was 4.55%,which was lower than 19.57%of the control group,and the difference was statistically significant(P<0.05).Conclusion:Compared with PSF,ILN is a more effective treatment for bone nonunion after traumatic fracture of extremities,which has the advantages of less intraoperative bleeding,shorter operation and recovery time,light stress reaction,better bone metabolism promotion effect and low complication rate.关键词
