Abstract
Objective:To investigate the value of dynamic contrast enhanced magnetic resonance imaging(DCE-MRI)and fat quantification in evaluating the lesion activity of sacroiliac joint in patients with ankylosing spondylitis(AS).Methods:84 patients with AS admitted to Yantai Laiyang Central Hospital from June 2021 to May 2024 were selected as the study objects.According to the test results of Bath ankylosing spondylitis disease activity index(BASDAI)and main laboratory indicators,the activity of sacroiliac joint lesions was evaluated,and according to this,the patients were divided into the active group(n=53)and the stable group(n=31).All patients underwent conventional MRI sequence,diffusion weighted imagin(DWI),DCE-MRI,and iteraterative decomposition of water and fat with echo asymmetry and least-squares estimation quantitation scans,the DCE-MRI quantization parameters,apparent diffusion coefficient(ADC)values and bone marrow fat fraction(FF)values of the two groups were compared.In addition,Pearson analysis was used to discuss the correlation of BASDAI index and each parameter,and receiver operating characteristic curve(ROC)was used to discuss the evaluation value of each parameter.Results:The rate constant(Kep),volume transfer constant(Ktrans),time to peak(TTP)and ADC values in the active period group were higher than those in the stable period group,and the FF value was lower than that in the stable period group,the differences were statistically significant(P<0.05).According to Pearson's analysis,Ktrans(r=0.455,P=0.000),Kep(r=0.233,P=0.033),ADC(r=0.371,P=0.001)were positively correlated with BASDAI index of AS patients,FF value(r=-0.281,P=0.010)was positively correlated with BASdai index of AS patients.ROC curve analysis showed that the sensitivity of Ktrans and FF were relatively high,which were 83.02%and 94.34%respectively.The specificity of Kep and TTP were both 93.55%,the specificity of Ktrans was 90.33%,and the specificity of other parameters was relatively low.Conclusion:DCE-MRI and IDEAL-IQ scan have certain reference value in the evaluation of lesion activity in AS patients,and clinical attention should be paid to the detection results of relevant quantitative parameters.关键词
