Abstract

Objective To evaluate the diagnostic value of serum 1,3-beta-D glucan(BDG)combined with metagenomic next-generation sequencing(mNGS)for Pneumocystis jirovecii pneumonia(PJP)in with non-human immunodeficiency virus(HIV)infected children.Methods A retrospective collection was conducted on 21 PJP children and 41 non PJP children from 4 hospitals.The clinical characteristics of PJP patients were summarized,and the diagnostic efficacy of PJP was evaluated using ser-um 1,3-beta-D glucan(BDG)combined with BALF-mNGS.Receiver operating characteristic(ROC)curves were drawn,and spearman analysis was used to analyze the correlation between the two group.Results The area under the BALF-mNGS curve of was 0.907(95%CI 0.772~0.963),and the area under the curve of serum BDG was 0.869(95%CI0.743~0.971).The optimal cut-off values for BALF-mNGS and serum BDG to distinguish whether PJP was infected were 12 sequence numbers(sensitivity 83.2%,specificity 94.3%)and 64.6 ng/L(sensitivity 79.2%,specificity 93.1%),respectively.Spearman correlation analysis showed a significant correlation between the mNGS sequences and serum BDG(r=0.718,P<0.001).When the number of mNGS sequences in BALF was≥12 and serum BDG was≥64.6 ng/L,the sensitivity was 95.2%(95%CI 74.1%-99.8%),the specificity was 85.3%(95%CI 70.1%-93.9%),the positive predictive value was 76.9%(95%CI 55.9%~90.2%),and the negative predictive value was 97.2%(95%CI 83.8%-99.9%)for joint diagnosis.Conclusion Early BALF mNGS com-bined with serum BDG is helpful for the diagnosis of PJP children with non-HIV infected.
