脑灌注CT影像参数对高龄缺血性脑血管病患者预后的评估价值
Evaluation Value of Cerebral Perfusion CT Imaging Parameters for Prognosis of Elderly Patients with Ischemic Cerebrovascular Disease

Abstract
Objective To explore the evaluation value of cerebral perfusion CT imaging parameters for prognosis of elderly patients with ischemic cerebrovascular disease.Methods 80 elderly patients with ischemic cerebrovascular disease admitted to our hospital from October 2020 to October 2024 were retrospectively analyzed.All patients underwent cerebral perfusion CT examination to obtain parameters such as cerebral blood flow(CBF),cerebral blood volume(CBV),mean transit time(MTT),and time to peak(TTP),and divided into the good prognosis group(mRS score ≤ 2)and the poor prognosis group(mRS score＞2).The CBF,CBV,MTT,and TTP parameters were compared between the two groups,and the correlation between each parameter and mRS score was analyzed.Receiver Operating Characteristic(ROC)curve was draw to analyze the evaluation value of various parameters for prognosis of patients with ischemic cerebrovascular disease.Results Statistical difference was found in the cerebral perfusion CT imaging parameters between the two groups(P＜0.05).The CBF and CBV of the good prognosis group were higher than those of the poor prognosis group,and the MTT and TTP were lower than those of the poor prognosis group(P＜0.05).Pearson correlation analysis showed that CBF(r=-0.648)and CBV(r=-0.522)were negatively correlated with mRS scores,while MTT(r=0.591)and TTP(r=0.439)were positively correlated with mRS scores(P＜0.05).ROC curve analysis results showed that the CBF,CBV,MTT and TTP had significant evaluation value for prognosis of patients with ischemic cerebrovascular disease(AUC＞0.7,P＜0.05).Conclusions The parameters of cerebral perfusion CT imaging are closely related to the prognosis of elderly patients with ischemic cerebrovascular disease,and have significant value in the evaluation of prognosis,which can be used as effective reference indicators for clinical evaluation of prognosis.关键词
