高分辨率超声引导下穿刺细胞学活检和免疫检查对甲状腺癌的诊断意义
Diagnostic Significance of High-Resolution Ultrasound-Guided Puncture Cy-tology Biopsy and Immunoassay in Thyroid Cancer

Abstract
Objective To study the diagnostic significance of high-resolution ultrasound-guided puncture cytology biopsy(UGB)and immunoassay in thyroid cancer.Methods 86 patients with suspected thyroid cancer admitted to our hospital from March 2022 to February 2024 were selected and given high-resolution UGB and immunoassay[using immunohistochemistry to detect galactoagglutinin-3,thyroid medullary peroxidase(TPO),and cytokeratin 19(CK-19)expression].Taking postoperative pathology results as the"gold standard",the results and efficacy of single and combined detection of high-resolution UGB and immunoassay in the diagnosis of thyroid cancer were analyzed.Results Among 86 patients with suspected thyroid cancer,postoperative pathology results showed 45 positive cases and 41 negative cases;high-resolution UGB showed 38 positive cases and 48 negative cases;immunoassay showed 37 positive cases and 49 negative cases;the combined detection showed 44 positive cases and 42 negative cases.The sensitivity and accuracy of combined detection in the diagnosis of thyroid cancer were 97.78％(44/45)and 98.84％(85/86),respectively,higher than 77.78％(35/45)and 84.88％(73/86)of high-resolution UGB,and 80.00％(36/45)and 88.37％(76/86)of immunoassay(P＜0.05).The missed diagnosis rate of combined detection was 2.22％(1/45),lower than 22.22％(10/45)of high-resolution UGB and 20.00％(9/45)of immunoassay(P＜0.05).The negative predictive value of combined detection was 97.62％(41/42),higher than 79.17％(38/48)of high-resolution UGB and 81.63％(40/49)of immunoassay(P＜0.05).No statistical difference was found in the positive predictive value among the three detection methods(P＞0.05).Conclusions High-resolution UGB combined with immunoassay has better diagnostic efficacy for thyroid cancer with complementary advantages,can improve the diagnostic sensitivity and accuracy,and reduce the missed diagnosis,which is worthy of clinical promotion.关键词
