Abstract
Objective To explore the application value of multi sequence magnetic resonance imaging(MRI)in the diagnosis of placental implantation(PAS).Methods 80 suspected PAS patients who underwent prenatal ultrasound examination at our hospital from July 2020 to August 2024 were selected as the study subjects.They underwent single shot fast spin echo(SSFSE)sequence,single shot planar echo imaging(SSEPI)sequence,and T2 weighted imaging(T2WI)sequence scanning.The postoperative pathological results were used as the"gold standard"for diagnosis to analyze MRI examination results and imaging manifestations,the diagnostic values of different sequence MRI scans in PAS were compared,and their consistency with the"gold standard"was analyzed.Results MRI examination detected 56 patients with PAS,including 22 cases of implantable PAS,22 cases of adhesive PAS,and 12 cases of penetrating PAS.The sensitivity,accuracy,and negative predictive value of SSFSE sequence in PAS diagnosis were higher than those of SSEPI and T2WI sequences,and SSEPI sequence was higher than T2WI sequence(P<0.05);No statistically significant difference was found in the diagnostic specificity and positive predictive value of PAS between different sequence MRI scans(P>0.05).The consistency test results showed that the diagnostic results of SSFSE with the"gold standard"was good(Kappa=0.772,P=0.000),the diagnostic consistency of SSEPI was acceptable(Kappa=0.499,P=0.000),and the diagnostic consistency of T2WI was poor(Kappa=0.118,P=0.077).Conclusions Multi sequence MRI examination has high diagnostic value in PAS,with SSFSE having the highest diagnostic efficiency,higher accuracy and consistency,which can provide effective reference for the diagnosis and treatment of PAS and is worthy of promotion and application.关键词
