128排(256层)CT诊断肺癌患者肿瘤分化及纵隔淋巴结转移的价值
128-row(256-slice)CT examination in the diagnosis of tumor differentiation and mediastinal lymph node metastasis in patients with lung cancer

Abstract
Objective:To investigate the diagnostic value of 128-row(256-slice)CT in tumor differentiation and mediastinal lymph node metastasis in patients with lung cancer.Methods:Ninty-two patients with lung cancer admitted to our hospital from June 2020 to December 2022 were selected as research objects.According to the results of pathological examination,they were divided into 29 cases of adenosquamous carcinoma,29 cases of adenocarcinoma and 34 cases of squamous carcinoma.In addition,92 healthy people examined during the same time were collected as the control group.All research subjects underwent 128-row CT examinations.The degree of lung cancer tissue differentiation,lymph node distribution and diagnostic accuracy in patients with lung cancer were compared.Results:The parameter skewness and the peak height of the mass distribution map of patients with low,moderate and high differentiation degree of lung cancer tissue decreased in turn(P<0.05).Mediastinal lymph nodes were more common in patients with lung cancer,and enlarged lymph nodes were more common;among them,adenosquamous carcinoma lymph nodes were the most significant,which were mostly distributed in groups 4,7 and 10.If the number of cancerous lymph nodes was small,the tumor was small,and the number of enlarged lymph nodes was less.Enlarged lymph nodes were more common in the mediastinum region of patients with large tumor diameter,which were mostly distributed in groups 4,7,and 10.The detection rate of enhanced CT for lung cancer types was 100%,and the diagnostic accuracy was 97.83%.ROC curve analysis showed that 128-row(256-slice)CT examination could accurately diagnose the degree of low,moderate and high differentiation in patients of tumors,and the sensitivity was 0.813,0.844 and 0.869,respectively;at the same time,128-row(256-slice)CT examination could accurately diagnose the size of lymph nodes of the patients as<5 mm,5~<10 mm and≥10 mm,and the sensitivity was 0.862,0.893 and 0.887 respectively,which had high diagnostic value(AUC>0.70).Conclusion:128-row(256-slice)CT examination can effectively determine the degree of tumor differentiation and lymph node distribution in patients with lung cancer,and the clinical correct diagnosis rate is higher,which can provide a reference basis for the comprehensive evaluation of patients'disease.关键词
