重视消化道组织机械吻合管理
Emphasizing the management of mechanical anastomosis in digestive tract tissues
摘要
Abstract
Anastomosis-related complications are closely associated with gastrointestinal surgery.Clinically, the causes are generally attributed to three factors including the surgeon, the patient, and the instruments, with the surgeon's factors being the most crucial.Apart from continually improving surgical skills based on a deep understanding of medical scientific theory, surgeons are also associated with another two factors patients and instruments, which are the subjects of treatment and the tools used, respectively.Regarding patients,in addition to inherent factors such as age and physical condition, surgeons should adjust and maintain the functions of various organs, including perioperative optimization and nutritional support,to help patients cope with surgical stress.Regarding instruments,in addition to the long-standing emphasis on the quality, the accuracy and the manipulability of instruments in clinical practice, the surgeon's handling of instruments is also crucial. The target of mechanical anastomosis is gastrointestinal tissue, which exhibits biphasic and viscoelastic properties.During compression and stapling, the tissue demonstrates hysteresis, stress relaxation, and creep, making it highly susceptible to damage that can impair healing. Therefore, surgeons should grasp the principles of mechanical anastomosis and the performance and usage methods of various instruments, and be familiar with the characteristics of gastrointestinal tissue to correctly use instruments during procedures, compressing and stapling in a manner that aligns with the tissue properties,to ensure the anastomosis in the most favorable state for tissue healing.关键词
