Predictive Value of Mean Platelet Volume/Lymphocyte Ratio Combined with Thromboelastography Parameters for Postoperative Lower Extremity Deep Vein Thrombosis in Patients with Traumatic Fracture

Abstract
Objective To explore the predictive value of the mean platelet volume/lymphocyte ratio(MPVLR)combined with thromboelastography(TEG)parameters for the occurrence of lower extremity deep vein thrombosis(LEDVT)after surgery in patients with traumatic fracture(TF).Methods A total of 155 TF patients who underwent surgical treatment at Nanjing University of Chinese Medicine Affiliated Nanjing Integrated Traditional Chinese and Western Medicine Hospital from January 2020 to April 2024 were included.Patients were divided into the LEDVT group(n=64)and the non-LEDVT group(n=91)based on the occurrence of postoperative LEDVT.MPVLR was calculated,and TEG parameters[including maximum amplitude(MA),α angle,coagulation time(K),and reaction time(R)]were measured.The relationships between MPVLR,TEG parameters,and postoperative LEDVT in TF patients were analyzed using multivariate unconditional logistic regression.The predictive value of MPVLR combined with TEG parameters was assessed using ROC curve analysis.Results Compared to the non-LEDVT group,the LEDVT group had higher MPVLR,MA,and α angle,and shorter K and R times(all P<0.05).High MPVLR,elevat-ed MA,and larger α angle were identified as independent risk factors for postoperative LEDVT in TF patients,while prolonged K time was an independent protective factor(all P<0.05).The area under the curve(AUC)for MPVLR combined with TEG parameters(MA,α angle,and K)in predicting postoperative LEDVT was 0.954,significantly higher than the AUCs of MPVLR(0.788),MA(0.783),α angle(0.815),or K(0.762)alone(all P<0.05).Conclusion MPVLR and TEG parameters(MA,α angle,and K)are associated with postoperative LEDVT in TF patients.MPVLR combined with TEG parameters provides high predictive value for LEDVT occurrence.关键词
