医护一体化延续护理模式在慢性伤口病人中的应用
Application of integrated medical-nursing continuity care model in patients with chronic wounds

Abstract
Objective:To explore the application effect of integrated medical-nursing continuity care model in patients with chronic wounds.Methods:A total of 100 patients with chronic wounds who visited the wound-stoma outpatient department of a tertiary grade A hospital in Shanxi province were selected as the research subjects from January to December 2023.The patients were divided into a control group and an experimental group by the random number table method,with 50 patients in each group.The control group received routine continuous care intervention,while the experimental group received integrated medical-nursing continuity care for intervention in addition to the control group's intervention.The wound healing time,wound healing status,pain score,quality of life score,patient satisfaction and medical expenses between two groups of patients were compared.Results:After intervention,the wound healing time of the experimental group[(54.46±12.50)d]was shorter than that of the control group[(64.78±7.95)d],the wound healing rate(94%)was higher than that of the control group(80%),the pain score[0(0,1)points]was lower than that of the control group[2(0,3)points],the quality of life score[(80.84±8.53)points]was higher than that of the control group[(74.28±11.05)points],the medical expenses[(1 824.40±583.66)yuan]were lower than those of the control group[(2 893.90±1 142.25)yuan],and the patient overall satisfaction(98.0%)was higher than that of the control group(84.0%),the differences were statistically significant(all P<0.05).Conclusions:The integrated medical-nursing continuity care model can effectively improve the wound outcomes effect of patients with chronic wounds,reduce pain,enhance patient satisfaction and quality of life,and reduce medical expenses.关键词
